Synthesis and bioevaluation of a new 68Ga-labelled niraparib derivative that targets PARP-1 for tumour imaging

Poly ADP-ribose polymerase (PARP) inhibitors prevent the repair of DNA single-strand breaks in cancer cells with abnormal homologous recombination, producing a synthetic lethal effect. Thus, PARP inhibitors have become clinically effective anticancer drugs. Labelling with radionuclides may extend the use of PARP inhibitors as tracers in nuclear medicine diagnostics, helping to stratify patients. In the present study, niraparib was selected as a skeleton molecule modified with 1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid (DOTA) and labelled with gallium-68 to obtain [⁶⁸Ga]Ga-DOTANPB with high radiochemical purity (>95 %). To verify the accuracy of the [⁶⁸Ga]Ga-DOTANPB structure, [^natGa]Ga-DOTANPB was also synthesized, and in vitro affinity experiments were performed, which revealed a high affinity for PARP-1 (IC₅₀ = 82.21 nM). [⁶⁸Ga]Ga-DOTANPB is hydrophilic and has good in vitro stability within 3 h. In in vitro experiments, [⁶⁸Ga]Ga-DOTANPB has a high uptake in HeLa cells and can enter the cell to target PARP-1. In coronal PET imaging of HeLa tumour-bearing mice, [⁶⁸Ga]Ga-DOTANPB showed significant radioconcentration at the tumour site at 0.5 h, 1 h, and 2 h. Biodistribution and autoradiography experiments revealed that [⁶⁸Ga]Ga-DOTANPB has obvious tumour uptake and can be significantly inhibited (3.37 ± 0.33 % ID/g vs. 2.50 ± 0.27 % ID/g, **P < 0.01), suggesting that it has PARP-1 specificity. Thus, these findings suggested that [⁶⁸Ga]Ga-DOTANPB may be a potential niraparib-based PET tracer for targeting PARP-1.

Pharmacokinetic Positron Emission Tomography Imaging of an Optimized CD38-Targeted ⁶⁸Ga-Labeled Peptide in Multiple Myeloma: A Pilot Study

Multiple myeloma (MM) is an incurable disease characterized by its clinical and prognostic heterogeneity. Despite conventional chemotherapy and autologous hematopoietic stem cell transplantation, the management of relapsed and refractory MM disease poses significant challenges, both medically and socioeconomically. CD38, highly expressed on the surface of MM cells, serves as a distinct tumor biological target in MM. Peptides offer advantages over antibodies, enabling precise tumor imaging and facilitating early tumor diagnosis and dynamic immunotherapy monitoring. In this study, we developed PF381, a CD38-targeted peptide, and investigated its role in diagnosis, biodistribution, and dosimetry through ⁶⁸Ga-labeling for preclinical evaluation in tumor-bearing models. We screened a microchip-based combinatorial chemistry peptide library to obtain the amino acid sequence of PF381. Affinity for human CD38 was evaluated by SPRi. PF381 was conjugated with DOTA for radiolabeling with ⁶⁸Ga, and the complex was characterized by HPLC. PET imaging was performed in murine tumor models after the administration of [⁶⁸Ga]Ga-DOTA-PF381. Biodistribution analysis compared CD38-positive H929 and CD38-negative U266 tumors, and human radiation dosimetry was estimated. Tumor sections were stained for CD38 expression. SPRi showed that PF381 had a high affinity for CD38 with a KD of 2.49 × 10^-8 M. HPLC measured a radiolabeling efficiency of 78.45 ± 7.91% for [⁶⁸Ga]Ga-DOTA-PF381, with >98% radiochemical purity. PET imaging revealed rapid and persistent accumulation of radioactivity in CD38-positive H929 tumors, contrasting with negligible uptake in CD38-negative U266 tumors. Biodistribution confirmed higher uptake in H929 tumors (0.75 ± 0.03%ID/g) vs U266 (0.26 ± 0.08%ID/g, P < 0.001). The kidney received the highest radiation dose (3.57 × 10^-02 mSv/MBq), with an effective dose of 1.41 × 10^-02 mSv/MBq. Immunofluorescence imaging supported PET and biodistribution findings. We developed a novel peptide targeting CD38 and proved that ⁶⁸Ga-labeled PF381 had rapid targeting and good tumor penetration capabilities. Therefore, ⁶⁸Ga-labeled PF381 could achieve high sensitivity in vivo imaging for CD38-positive hematological malignancies.

Assessment of PD‐L1 Expression in Non‐Small Cell Lung Cancers Using [⁶⁸Ga]Ga‐DOTA‐WL12 PET/CT

Abstract:

Assessing programmed death ligand‐1 (PD‐L1) expression in non‐small cell lung cancer (NSCLC), particularly in metastatic cases, remains challenging. In this study, surface plasmon resonance (SPR) analysis and [68Ga]Ga‐DOTA‐WL12 micro‐PET/CT imaging are performed. [68Ga]Ga‐DOTA‐WL12 PET/CT and [18F]FDG PET/CT are performed on a cohort of 20 patients with NSCLC. Semi‐quantitative assessments include SUVmax, metabolic tumor volume (MTV), total lesion glycolysis (TLG), and target‐to‐background ratio (TBR). DOTA‐WL12 exhibits robust PD‐L1 binding with a KD value of 0.2 nM. Subsequent human studies reveal significant correlations between PD‐L1 expression and the [68Ga]Ga‐DOTA‐WL12 SUVmax in primary and metastatic lesions, surpassing the [18F]FDG results (r = 0.8889, p <0.0001 vs r = 0.0469, p = 0.8127). Notably, [68Ga]Ga‐DOTA‐WL12 imaging discerned SUVmax and TBR differences between PD‐L1 TPS ≤1% and PD‐L1 TPS > 1% groups (p all <0.001). In an NSCLC patient with brain metastases, [68Ga]Ga‐DOTA‐WL12 shows a SUVmean of 0.04 in the brain background, with TBR values of 17 and 23, underscoring its potential for detecting brain metastases. The study provides initial evidence for the clinical utility of [68Ga]Ga‐DOTA‐WL12 PET/CT for lesion detection, immunotherapy selection, and therapeutic efficacy evaluation in PD‐L1‐expressing NSCLC, demonstrating its potential as a valuable tool in NSCLC research and management.

RLS Radiopharmacies Partners with Eckert & Ziegler to Expand Production of Gallium-68-Based Radiopharmaceuticals

RLS expands rCDMO capabilities with Eckert & Ziegler's GalliaPharm® generators for improved patient diagnostics RLS Radiopharmacies, America’s only Joint Commission-accredited radiopharmacy network, has expanded its radiopharmaceutical contract development and manufacturing (rCDMO) capabilities by entering into a strategic agreement with global isotope technology leader Eckert & Ziegler (ISIN DE0005659700, SDAX). Under the partnership, all 31 of RLS’s radiopharmacies will be equipped with Eckert & Ziegler’s advanced GalliaPharm® generators, which the radiopharmacy network will utilize to produce high-quality, life-enhancing Gallium-68-based (Ga-68) radiopharmaceuticals. Ga-68 is crucial for Positron Emission Tomography (PET) imaging and plays a pivotal role in the accurate diagnosis and subsequent treatment planning for patients with neuroendocrine tumors and prostate cancer. “To help meet the ever-increasing demand for precision diagnostics in cancer treatment, we are collaborating with Eckert & Ziegler to place advanced Ga-68 radiolabeling technology into the hands of all our skilled radiopharmacists nationwide," said RLS CEO Stephen Belcher. “A powerful diagnostic isotope, Gallium has an extremely short half-life, necessitating the need for Ga-68-based radiopharmaceuticals to be produced within minutes of patient dosing. By bringing Eckert & Ziegler’s GalliaPharm® generators into our network, we are closing the patient care gap in many parts of the U.S. This signifies a major leap forward in our commitment to advancing healthcare and improving patient outcomes through cutting-edge radiopharmaceuticals." To date, RLS has installed Eckert & Ziegler’s GalliaPharm® generators within most of its locations and anticipates the remaining radiopharmacies will be operational by the end of June. All of RLS’s radiopharmacies house cutting-edge clean rooms and are fully equipped and aligned to USP <797>, USP <825> and ISO 14644-1 compliance standards, which guarantees Ga-68-radiolabeled products are produced to the highest standard possible. Additionally, each location is led by experienced nuclear pharmacists, who, along with nuclear technicians, in-house couriers and a robust delivery fleet, ensure every order is meticulously prepared, dispensed and distributed. RLS is also investing in the future by building a national distributed rCDMO network. 80,000 sq. ft. of space across eight of its pharmacies will be used for radiopharmaceutical development, manufacturing and the production of medical isotopes such as Gallium-68, Actinium-225 and Lutetium-177. RLS now offers Ga-68-based radiolabeling, custom compounding, patient dose preparation and last-mile delivery. The generator offers a low-cost alternative for the radiolabeling of biomolecules with Gallium-68 in PET, an imaging examination method used to detect the presence or absence of diseased tissue. PET Imaging is primarily used in the diagnosis of cancer, heart attacks or neurological diseases. Radioisotopes such as Fluorine-18 can be used alternatively but require investments of millions of dollars in large-scale equipment (cyclotrons). The Ge-68/Ga-68 generator on the other hand, is an easily transportable, small metal box and can be obtained much more cost-efficient, leading to cost reductions in nuclear medicine clinics and practices while increasing flexibility.
