⁵²Mn-labelled Beta-cyclodextrin for Melanoma Imaging: A Proof-of-concept Preclinical Study

BACKGROUND/AIMAs prostaglandin E2 (PGE2) and its receptors (EP2) are over-expressed on tumor cells and microenvironment, radiolabeled cyclodextrins targeting such biomolecules are valuable vector candidates in molecular cancer diagnostics. Using experimental melanoma models, we evaluated the in vivo imaging behavior of novel Manganese-52-labeled (⁵²Mn) randomly methylated beta-cyclodextrin ([⁵²Mn]Mn-DOTAGA-RAMEB) and compared it with the following well-established tumor-specific probes: melanocortin-1 receptor (MC1-R)-affine [⁶⁸Ga]Ga-DOTA-NAPamide and PGE2 selective [⁶⁸Ga]Ga-DOTAGA-RAMEB cyclodextrin.MATERIALS AND METHODSPost-injection of [⁶⁸Ga]Ga-DOTA-NAPamide, [⁶⁸Ga]Ga-DOTAGA-RAMEB, and [⁵²Mn]Mn-DOTAGA-RAMEB into MC1-R positive B16F10 melanoma-bearing mice, tumor radio-pharmaceutical uptake was quantified in vivo and ex vivo using preclinical positron emission tomography (PET) and high-performance gamma counter.RESULTSAlthough all tracers performed well in tumor identification, the highest standardized uptake values were detected in the [⁶⁸Ga]Ga-DOTA-NAPamide scans. Corresponding to the ex vivo data, meaningful [⁵²Mn]Mn-DOTAGA-RAMEB accumulation 1 h post-injection confirmed the tumor-targeting potential of the tracer. Temporal changes in PGE2/EP2 expression of the neoplasms may explain the significant differences observed between the tumor uptake of the two cyclodextrin probes and that of the ⁵²Mn-labelled compound measured 1 h, 4 h, and 3 days post-injection (p≤0.01, p≤0.05).CONCLUSIONAlthough further pharmacokinetical optimization may be required, ⁵²Mn-labelled cyclodextrin holds potential in melanoma diagnostics and the PET-based longitudinal assessment of tumor-associated PGE2/EP2 expression.

Granzyme B PET/CT Imaging Evaluates Early Response to Immunotherapy in Gastric Cancer

In several malignancies, only a limited number of patients respond to immune checkpoint inhibitors. Predicting and monitoring responses to these inhibitors represent an unmet clinical need. Here, we developed a PET/CT probe targeting granzyme B, [⁶⁸Ga]Ga-NOTA-Gly-Gly-Gly-Ile-Glu-Pro-Asp-CHO (GSI), and aimed to investigate whether it can be used to monitor the effects of immune checkpoint inhibitors early in the course of therapy. Methods: Seventy-two patients with gastric cancer (stages III-IV) were recruited for [⁶⁸Ga]Ga-NOTA-GSI PET/CT imaging after 2 or 3 cycles of the immunotherapy, and 40 patients were included in the final analysis. The SUV_max of primary tumors (SUV_max-t), SUV_max of metastatic lymph nodes (SUV_max-LN), and SUV_max of normal tissues (liver and blood pool) were measured, and their target-to-liver background ratio (TLR) and target-to-blood background ratio (TBR) were denoted for primary tumors as TLR_tumor and TBR_tumor and for metastatic lymph nodes as TLR_LN and TBR_LN, respectively. The treatment responses were assessed within 1 wk after full-course treatment according to RECIST version 1.1. Wilcoxon rank-sum tests were used to compare the PET/CT parameters between responders and nonresponders. Receiver operating characteristic curve analysis was used to assess the diagnostic efficacy of [⁶⁸Ga]Ga-NOTA-GSI PET/CT parameters in identifying responders. Two-tailed P value of less than 0.05 was considered statistically significant. Results: We found that SUV_max-t, TLR_tumor, TBR_tumor, SUV_max-LN, and TBR_LN were higher in responders than in nonresponders (2.49 ± 0.58 vs. 1.55 ± 0.48, P = 0.000; 2.24 ± 0.48 vs. 1.74 ± 0.67, P = 0.007; 1.38 ± 0.43 vs. 0.90 ± 0.23, P = 0.000; 2.24 ± 0.99 vs. 1.42 ± 0.55, P = 0.003; and 1.28 ± 0.68 vs. 0.83 ± 0.32, P = 0.012, respectively). According to receiver operating characteristic curve analysis, the area under the curve for SUV_max-t, TBR_tumor, TLR_tumor, SUV_max-LN, TLR_LN, and TBR_LN was 0.886, 0.866, 0.746, 0.772, 0.648, and 0.731, respectively. The threshold of SUV_max-t was 2.05, and its sensitivity and specificity were 81.0% and 84.2%, respectively. In addition, multivariate logistic regression indicated that TBR_tumor was an independent predictor of treatment response (P = 0.03). Conclusion: Our results indicated that [⁶⁸Ga]Ga-NOTA-GSI PET/CT is a promising tool for predicting early response to combined immunotherapy in gastric cancer patients.

⁶⁸Ga/¹⁷⁷Lu-Labeled Theranostic Pair for Targeting Fibroblast Activation Protein with Improved Tumor Uptake and Retention

Fibroblast activation protein (FAP) is specifically expressed on cancer-associated fibroblasts in over 90% of tumors and is considered a promising target for cancer theranostics. Here, we developed a novel tracer, DOTA-FAPT, and labeled it with gallium-68 and lutetium-177 as a theranostic pair. [⁶⁸Ga]Ga/[¹⁷⁷Lu]Lu-FAPT exhibited high stability and hydrophilicity, as well as strong affinity to the FAP target. Micro-PET/CT imaging revealed that [⁶⁸Ga]Ga-FAPT exhibited significantly increased uptake in tumors and extended retention in A549-FAP and U87MG tumor xenografts as compared to [⁶⁸Ga]Ga-FAPI-04, demonstrating favorable pharmacokinetic characteristics in vivo. Therapeutic studies showed that [¹⁷⁷Lu]Lu-FAPT had higher tumor accumulation compared to [¹⁷⁷Lu]Lu-FAPI-04, leading to stronger tumor growth inhibition. The first-in-human evaluation also revealed that [⁶⁸Ga]Ga-FAPT has good in vivo distribution and superior diagnostic efficacy on primary and lymph node metastases in a patient with lung cancer. Our encouraging results suggest that ⁶⁸Ga/¹⁷⁷Lu-labeled DOTA-FAPT is a theranostic pair with broad application prospect.

RLS expands rCDMO capabilities with Eckert & Ziegler's GalliaPharm® generators for improved patient diagnostics RLS Radiopharmacies, America’s only Joint Commission-accredited radiopharmacy network, has expanded its radiopharmaceutical contract development and manufacturing (rCDMO) capabilities by entering into a strategic agreement with global isotope technology leader Eckert & Ziegler (ISIN DE0005659700, SDAX). Under the partnership, all 31 of RLS’s radiopharmacies will be equipped with Eckert & Ziegler’s advanced GalliaPharm® generators, which the radiopharmacy network will utilize to produce high-quality, life-enhancing Gallium-68-based (Ga-68) radiopharmaceuticals. Ga-68 is crucial for Positron Emission Tomography (PET) imaging and plays a pivotal role in the accurate diagnosis and subsequent treatment planning for patients with neuroendocrine tumors and prostate cancer. “To help meet the ever-increasing demand for precision diagnostics in cancer treatment, we are collaborating with Eckert & Ziegler to place advanced Ga-68 radiolabeling technology into the hands of all our skilled radiopharmacists nationwide," said RLS CEO Stephen Belcher. “A powerful diagnostic isotope, Gallium has an extremely short half-life, necessitating the need for Ga-68-based radiopharmaceuticals to be produced within minutes of patient dosing. 为打造全球ADC/RDC未来产业高地，成都医学城拟于2024年4月18日-19日举办首届未来XDC新药大会！大会以“共创偶联药物产业未来”为主题，由同写意策划，其中设未来核药会议系列论坛，“核药全流程开发（讲习班）、分会六：拥抱核药产业未来、分会七：核药源头创新与前沿技术、分会八：核药上下游产业链论坛”将围绕核药展开全面深入探讨，欢迎报名！二代放射性药物在肿瘤领域的商业成功重新点燃了投资者对该领域的兴趣，相关交易和融资的也应声激增。预计到2032年全球核药市场将达到103亿美金规模，行业增长率高达8.85%。全身性放射性药物作为癌症的治疗方法最初于 1940 年代使用，当时医生利用放射性碘来治疗甲状腺癌。然而，早期放疗缺乏效力以及由于缺乏特异性而产生的副作用，科学家和临床医生一直致力于通过将放射性同位素与靶向部分（如肿瘤特异性小分子、肽或抗体）联系起来来开发更安全、更有效的药物。商业化的抗体靶向放射治疗药物的出现始于本世纪初，2002 年和 2003 年，Zevalin（ibritumomab tiuxetan;现在由 Acrotech Biopharma 销售）和 Bexxar（tositumomab 和 iodine-131 [I-131] tositumomab;由葛兰素史克公司开发）分别被批准用于治疗非霍奇金淋巴瘤。尽管临床疗效优异，但一些关键挑战——包括对专业设施、设备和生产和处理专业知识的需求——限制了他们的商业成功。Zevalin 和 Bexxar 的销量在推出后的十多年一直很低，导致 Zevalin 的所有权多次发生变化，Bexxar 被撤出市场。不同的是，第二代放射性药物Xofigo（镭-223[Ra-223]二氯化物;拜耳，2013 年获批）、Lutathera（镥-177 [Lu-177] dotatate，2018 年批准）和 Pluvicto（镥-177 [Lu-177] vipivotide tetraxetan;诺华，2022年获批），由于采取了一些应对措施，已取得了一定的商业成功。事实上，被批准用于治疗转移性去势抵抗性前列腺癌的Pluvicto已经有望成为重磅炸弹，诺华在2023年全年的销售额达到9.8亿美元。与现有的标准疗法相比，更优的疗效、肿瘤学临床实践的发展、放射肿瘤学家的增多、以及使用具有更高稳定性和安全性的放射性同位素等等新的措施的使用，这些汇聚在一起，使得核药领域焕发了“第二春”。也正是更有效、更安全、更易于使用的具有强大商业潜力的药物的出现激发了投资者对该领域的兴趣，导致了最近一系列的许可合作伙伴关系、并购（M&A）和风险投资（VC）融资。下文总结评估了过去 5 年里放射性药物领域的交易和投资格局，涵盖治疗、诊断和治疗诊断学等。1放射性药物交易过去 5 年中，放射性药物领域共达成了 86 项战略交易，其中大部分是合作/共同开发（n = 26）和基于许可的协议（n = 26）。从2018年到2021年，交易数量相对稳定。然而，在 2022 年，达成的交易数量与 2021 年相比几乎增加了两倍（分别为 n = 25 和 n = 9;图1a）。值得注意的是，在临床前阶段（n = 20）的资产交易数量最多，主要是由具有现有核医学能力和专业知识的公司（如Fusion Pharmaceuticals和Novartis）推动的，这些公司正在寻求扩大其放射性药物管线。许多交易也发生在II期临床药物（n = 18）;这些主要是基于放射性诊断药物的许可和收购交易，用于识别最适合作公司治疗资产的患者（图1a）。图 1 |生物制药赞助的放射性药物领域交易分配：2018-2023年。a，每年按阶段划分的交易数量。b，按适应症、按用途分列的交易数量。数据来源：Cortellis。*至2023年10月3日。该领域活跃的整合者包括全球生物制药公司（如诺华和拜耳）和专门的核制药公司（如Fusion Pharmaceuticals和Curium Pharma），交易活动遍及诊断、治疗和治疗药物。放射性诊断药物在该领域的交易总数中占据了相当大的比例（~41%），其次是治疗诊断学（~32%）和治疗学（~27%;图1b）。虽然肿瘤学在治疗诊断学和治疗学交易领域占据主导地位（分别占交易的93%和100%），但诊断领域交易的最大部分是神经病学（54%），其次是肿瘤学（31%）。在分析的期间内，有 17 笔交易在公告时披露了财务条款（~20%）。在开发和模式的所有阶段，商业前交易的平均潜在总价值为8.34亿美元（前期2.84亿美元，里程碑5.5亿美元）（图2a）。与诊断资产的平均总交易价值（3900万美元;n = 4），治疗诊断学（n = 4）和治疗（n = 9）资产的潜在交易价值要高得多，分别为 7.29 亿美元和 10.26 亿美元。治疗诊断资产的平均前期交易价值是治疗资产的~2倍，这可能归因于前者的临床和监管风险较低。当按资产成熟度考虑交易价值时，随着技术在临床上的进展，临床阶段的资产价值不断增加，但与早期临床资产相比，发现阶段的交易实现了更高的总交易价值（图2b）。在发现阶段达成的交易的大部分（97%）价值在于里程碑付款，这反映了这些基于合作的交易在放射性药物疗法的发现、开发和商业化方面的高风险、高回报性质。图 2 |生物制药赞助的放射性药物领域的交易价值：2018-2023年*。a，按方式分列的具有披露条款的交易价值。b，按阶段划分的具有披露条款的交易价值。数据来源：Cortellis。*至2023年10月3日。商业阶段的交易主要是对拥有被FDA批准上市产品公司的收购。值得注意的是，诺华在2018年支付了39亿美元的预付款收购了Advanced Accelerator Applications，随后FDA批准了其神经内分泌肿瘤（NET）诊断Netspot（镓[ga-68]dotatate试剂盒），几乎同时FDA批准了治疗药物Lutathera。2公司融资 2018 年以来，放射性药物领域的融资数量（n = 54）一直在稳步增加，主要集中在癌症适应症上（n = 45;图 3）。治疗学是最活跃的融资方式（50%），其次是治疗（34%）和诊断（16%），大部分融资发生在早期，分别在A轮（56%）和B轮（32%）。随着兴趣和临床开发的不断提高，融资水平也在上升;2023 年前三季度 9 次融资的平均融资额为 5100 万美元，是 2018 年全年的 ~2 倍，4 次融资的 2600 万美元（图 3）。图 3 |放射性药物领域风险资本支持的私人融资：2018-2023年*。融资范围从A轮到D轮，数据来源：Pitchbook。*至2023年10月3日。3上市公司业绩虽然对放射性药物的热情导致交易增加，但自 2023 年初起在美国主要交易所（纽约证券交易所或纳斯达克）上市的五家放射性药物公司——Fusion Pharmaceuticals、Lantheus Medical Imaging、POINT Biopharma、Perspective Therapeutics 和 Actinium Pharmaceuticals——的表现喜忧参半。从年初到 2023 年 10 月 3 日，他们的表现从 POINT Biopharma 的股价上涨 71% 到 Actinium Pharmaceuticals 的股价下跌 43% 不等。五家公司中位数公司Perspective Therapeutics的股价上涨了11%，远高于纳斯达克生物技术指数，纳斯达克生物技术指数在同一时间段内的回报率为-7%。公共放射性制药公司平均表现积极的一个可能的解释是，围绕私人市场模式蔓延到公开市场的兴奋。强化这一理论的是RayzeBio的首次公开募股（IPO），这是一家放射性药物治疗公司，其主要资产是actinium-225（Ac-225）dotatate，处于治疗胃肠胰神经内分泌瘤的III期开发阶段。该公司于 2023 年 9 月 15 日在纳斯达克上市时筹集了 3.58 亿美元，上市后的估值接近 10 亿美元。这是今年迄今为止第二大生物技术 IPO，仅次于 Acelyrin 的 5.4 亿美元融资，是 2023 年医疗保健 IPO 平均规模的 ~2.9 倍。截至 2023 年底，两家上市放射性药物公司已达成收购协议，另外两家大型制药公司加入了诺华和拜耳的行列。2023 年 12 月 26 日，RayzeBio 与百时美施贵宝达成最终合并协议，以 41 亿美元收购，比 RayzeBio 当时的股价溢价 134%。一天后，礼来宣布以 14 亿美元完成对 POINT Biopharma 的收购，比 POINT 在 2023 年 10 月 3 日宣布收购前一天的收盘价溢价 87%。大型制药公司愿意为放射性药物公司支付高额溢价，这进一步表明了人们对该领域的兴趣日益浓厚。— 总结 — 随着大量投资涌入早期放射性药物公司，预计随着这些新技术的成熟，制药公司将继续其许可和并购活动。此外，随着对个性化医疗的日益重视，治疗诊断学可能会继续引起人们的浓厚兴趣；放射性药物在肿瘤学以外的领域的使用可能会带来更多的公司和风险投资公司。事实上，神经病学在诊断交易方面处于领先地位，因为放射性药物具有可视化大脑和中枢神经系统其他部分的独特能力。总体而言，随着交易的继续，放射性药物领域充满了期待和高期望。近期直播推荐:
