The positive association between the atherogenic index of plasma and the risk of new-onset hypertension: a nationwide cohort study in China.

BACKGROUND:

The atherogenic index of plasma (AIP) is a novel metabolic biomarker of atherosclerosis. Nevertheless, the association between the AIP and new-onset hypertension has not been elucidated in the Chinese population.

METHODS:

Prospective data were obtained from 3150 participants aged ≥ 18 years in the China Health and Nutrition Survey from 2009 to 2015. The AIP is a logarithmically transformed ratio of triglycerides to high-density lipoprotein cholesterol in molar concentration. Cox regression analysis was used to determine the association of AIP index with new-onset hypertension.

RESULTS:

After the six-year follow-up, 1054 (33.4%) participants developed new-onset hypertension. The participants were divided into AIP quartile groups (Q1-Q4). Compared with those in Q1, subjects in Q3-4 had nearly 1.35 times the risk of new-onset hypertension after full adjustment [Q3: hazard ratio (HR): 1.35, 95% confidence interval (CI): 1.13-1.62; Q4: HR: 1.35, 95% CI: 1.13-1.64]. The risks of new-onset hypertension were nearly 1.30 times higher in subjects in Q2-4 than in subjects in Q1 (p < .01) after the full adjustment when we excluded subjects with diabetes and/or chronic kidney diseases. There was a significant difference [HR (CI): 1.27 (1.04-1.54) vs. 0.90 (0.69-1.18)] when subjects were divided into two groups according to body mass index (BMI) level (<24 vs. ≥24 kg/m²).

CONCLUSIONS:

The present study suggested that individuals with a higher AIP index are associated with new-onset hypertension, independent of kidney function and glucose levels. The association was stronger in subjects with normal BMI, which may provide early screening of metabolomics in hypertension prevention.

Development and external validation of a nomogram for the early prediction of acute kidney injury in septic patients: a multicenter retrospective clinical study.

Background and purpose: Acute kidney injury (AKI) is a common serious complication in sepsis patients with a high mortality rate. This study aimed to develop and validate a predictive model for sepsis associated acute kidney injury (SA-AKI). Methods: In our study, we retrospectively constructed a development cohort comprising 733 septic patients admitted to eight Grade-A tertiary hospitals in Shanghai from January 2021 to October 2022. Additionally, we established an external validation cohort consisting of 336 septic patients admitted to our hospital from January 2017 to December 2019. Risk predictors were selected by LASSO regression, and a corresponding nomogram was constructed. We evaluated the model's discrimination, precision and clinical benefit through receiver operating characteristic (ROC) curves, calibration plots, decision curve analysis (DCA) and clinical impact curves (CIC) in both internal and external validation. Results: AKI incidence was 53.2% in the development cohort and 48.2% in the external validation cohort. The model included five independent indicators: chronic kidney disease stages 1 to 3, blood urea nitrogen, procalcitonin, D-dimer and creatine kinase isoenzyme. The AUC of the model in the development and validation cohorts was 0.914 (95% CI, 0.894-0.934) and 0.923 (95% CI, 0.895-0.952), respectively. The calibration plot, DCA, and CIC demonstrated the model's favorable clinical applicability. Conclusion: We developed and validated a robust nomogram model, which might identify patients at risk of SA-AKI and promising for clinical applications.

Krüppel-like factor 4 modulates the miR-101/COL10A1 axis to inhibit renal fibrosis after AKI by regulating epithelial-mesenchymal transition.

Acute kidney injury (AKI) can progress to renal fibrosis and chronic kidney disease (CKD), which reduces quality of life and increases the economic burden on patients. However, the molecular mechanisms underlying renal fibrosis following AKI remain unclear. This study tested the hypothesis that the Krüppel-like factor 4 (KLF4)/miR-101/Collagen alpha-1X (COL10A1) axis could inhibit epithelial-mesenchymal transition (EMT) and renal fibrosis after AKI in a mouse model of ischemia-reperfusion (I/R)-induced renal fibrosis and HK-2 cells by gene silencing, overexpression, immunofluorescence, immunohistochemistry, real-time quantitative PCR, Western blotting, dual-luciferase reporter assay, fluorescence in situ hybridization (FISH) and ELISA. Compared with the Sham group, I/R induced renal tubular and glomerular injury and fibrosis, and increased the levels of BUN, serum Scr and neutrophil gelatinase-associated lipocalin (NGAL), Col10a1 and Vimentin expression, but decreased E-cadherin expression in the kidney tissues of mice at 42 days post-I/R. Similarly, hypoxia promoted fibroblastic morphological changes in HK-2 cells and enhanced NGAL, COL10A1, Vimentin, and α-SMA expression, but reduced E-cadherin expression in HK-2 cells. These pathological changes were significantly mitigated in COL10A1-silenced renal tissues and HK-2 cells. KLF4 induces miR-101 transcription. More importantly, hypoxia upregulated Vimentin and COL10A1 expression, but decreased miR-101, KLF4, and E-cadherin expression in HK-2 cells. These hypoxic effects were significantly mitigated or abrogated by KLF4 over-expression in the HK-2 cells. Our data indicate that KLF4 up-regulates miR-101 expression, leading to the downregulation of COL10A1 expression, inhibition of EMT and renal fibrosis during the pathogenic process of I/R-related renal fibrosis.

Renalys Pharma Submits an IND Application for a Phase III Clinical Trial of sparsentan for IgA Nephropathy in Japan

TOKYO, April 7, 2024 /PRNewswire/ -- Renalys Pharma, Inc. Paxlovid中文全名为“奈玛特韦片/利托那韦片组合包装”，是3CL蛋白酶抑制剂与低剂量利托那韦的复方制剂，属于小分子口服新冠药，由辉瑞公司生产。从2021年12月，该药获得FDA的紧急使用授权，用于治疗非住院、具有发展成重症疾病高风险轻中度新冠患者，包括了12岁以上儿童以及成人。2022年2月11日，Paxlovid获中国药监局附条件进口批准，用于治疗成人伴有进展为重症高风险因素的至中度新冠患者。然而，近日《新英格兰医学杂志》（NEJM）发表了辉瑞公司EPIC-SR临床试验结果[1]，结果令人颇感意外。其显示在已接种疫苗且存在重症危险因素或未接种疫苗且不存在危险因素的Covid-19患者中，Paxlovid在症状持续缓解等主要终点方面疗效与安慰剂无显著差异。EPIC-SR临床试验结果该试验共招募近1300人：参与者在随机化前5天内出现症状，或为①全程接种疫苗且存在重症风险因素，或为②未接种疫苗（或在过去一年内未接种过Covid-19疫苗）且不存在危险因素。参与者接受为期5天的Paxlovid或安慰剂治疗，主要终点为症状持续缓解时间。57％的参与者接种过Covid-19疫苗，50％有重症危险因素。参与者的中位年龄为42岁，≥65岁者仅占5％。除肥胖、吸烟和高血压之外，其他重症Covid-19危险因素并不常见，如患有心脏病或肺病的参与者比例不足2％。在这一风险相对较低人群中，Paxlovid组和安慰剂组至主要终点的时间相似，其中位数分别为12天和13天。虽然Paxlovid组因Covid-19住院或去世的数量少于安慰剂组（5/654 [0.8％] vs. 10/634 [1.6％]，唯一死亡发生在安慰剂组），但差异并不显著。所有目标症状和体征持续缓解的时间Paxlovid并不是新冠万能药2021年，评估Paxlovid治疗高危Covid-19患者的EPIC-HR试验结果显示，Paxlovid能够将住院或死亡风险降低89％[2]。基于该项试验结果，Paxlovid成为非住院Covid-19患者的一线治疗方案。实验结果公布后，得益于“89%”这个惊人的数字，Paxlovid从此被很多人封为神药。但是，任何一款药物都不能过于神化，Paxlovid也是如此。根据目前的研究来看，Paxlovid具有诸多局限性。对于没有基础疾病的普通人来说，Paxlovid并没有太好的治疗效果。EPIC-HR试验针对的是高风险人群，例如伴有高龄、慢性肾脏疾病、糖尿病、心血管疾病、慢性肺病等重症高风险因素。2022年4月29日，辉瑞对外公布了一项研究其口服新冠药Paxlovid用于暴露后预防的II/III期临床（EPIC-PEP）研究情况，结果显示该药用于成人暴露后预防效果不佳[3]。结合EPIC-SR试验结果，我们应该更全面地评估Paxlovid的受益人群，同时在未来应采用更加标准化的试验设计和研究终点，以更好地比较结果。Paxlovid的“致命缺陷”Paxlovid并非一种药物，而是一款组合药物，包含奈玛特韦和利托那韦。其中利托那韦是一款“老药”，之前常用作抗艾滋病治疗。利托那韦的作用在于，降低肝脏的清除能力，减缓奈玛特韦的分解，从而增加活性抗病毒剂奈玛特韦的浓度。如果患者患有其他疾病，需服用多种药物，在利托那韦的抑制下，不仅减缓了奈玛特韦的分解，同时也减缓了其他药物在体内的分解，这是Paxlovid的致命缺点。根据FDA发布的《医生开具Paxlovid处方患者资格筛查清单》，129种药物需要值得关注，其中高血压用药阿呋唑嗪、抗心律失常药物胺碘酮、治疗痛风的秋水仙碱等35种药物被列为“严禁共同服用”，94种被描述为“避免共同服用，或对剂量进行调整，并进行必要的用药监控”，包含安非他酮、酮康唑、克拉霉素、红霉素等一些常见的抗抑郁、抗真菌、抗病毒、止疼类等药物。需要服用Paxlovid的患者往往是有着重症、住院或死亡风险的人群，包括老人、基础疾病患者等，而他们往往都会服用多种其他药物。因此，Paxlovid在临床上的使用应务必审慎。参考文献[1]https://www-nejm-org.libproxy1.nus.edu.sg/doi/10.1056/NEJMoa2309003[2]https://www.pfizer.com/news/press-release/press-release-detail/pfizers-novel-covid-19-oral-antiviral-treatment-candidate[3]https://www.pfizer.com/news/press-release/press-release-detail/pfizer-announces-additional-phase-23-study-results免责声明“药渡”公众号所转载该篇文章来源于其他公众号平台，主要目的在于分享行业相关知识，传递当前最新资讯。图片、文章版权均属于原作者所有，如有侵权，请及时告知，我们会在24小时内删除相关信息。微信公众号的推送规则又双叒叕改啦，如果您不点个“在看”或者没设为"星标"，我们可能就消散在茫茫文海之中~点这里，千万不要错过药渡的最新消息哦！
