Our team is committed to the seamless execution of our clinical development programs and, if successful, we believe each holds the potential to drive significant value in the near and long term.” Metastatic Pancreatic Ductal Adenocarcinoma Program Commenced enrollment and dosing in DURIPANC Phase 1b/2 study combining Ampligen with AstraZeneca’s anti-PD-L1 immune checkpoint inhibitor Imfinzi® (durvalumab) for the treatment of late-stage pancreatic cancer; Announced that the first dose level was generally well-tolerated in the DURIPANC Phase 1b/2 study of Ampligen and Imfinzi as a combination therapy for late-stage pancreatic cancer; and Reported positive preliminary data from Phase 1b/2 study of Ampligen and Imfinzi as a combination therapy for late-stage pancreatic cancer. Phase 2 Locally Advanced Pancreatic Adenocarcinoma Program Received authorization from the Erasmus Medical Center Ethics Committee to open a European site for the ongoing Phase 2 study (“AMP-270”) of Ampligen as a therapy for locally advanced pancreatic cancer; and Announced the publication of new positive data analysis from a long-term Early Access Program studying Ampligen for the treatment of advanced pancreatic ductal adenocarcinoma. The Company sought FDA guidance on expansion of inclusion criteria and treatment arms, and then subsequently amended the study protocol. The study is recruiting patients. These adjustments are also expected to result in substantial reductions in clinical costs. Phase 2 Recurrent Ovarian Cancer Program Reported positive top-line, protocol-planned interim report data from the study of Ampligen combined with pembrolizumab for the treatment of recurrent ovarian cancer. Phase 2 Post-COVID Conditions Program Reported positive topline results from the Company’s Phase 2 study evaluating the efficacy and safety of Ampligen as a potential therapeutic for people with the post-COVID condition of fatigue (“AMP-518”); and Reported an analysis of the AMP-518 clinical trial, based upon statistically significant data, which supports the Company’s belief in Ampligen as a potential therapeutic for people with the moderate-to-severe post-COVID-19 condition of fatigue, and that this would be the likely subject population for AIM’s planned follow-up clinical trial. Further, the Company was also granted two important patents covering Ampligen for the treatment of: Endometriosis, a painful chronic condition that affects nearly 10% of women of reproductive age, or approximately 6.5 million women in the United States. This patent was granted in the United States. The Post-COVID Condition of Fatigue. This method and compositions patent was granted in the Netherlands. Additionally, AIM successfully completed cGMP manufacturing of 9,042 clinical vials of Ampligen. 1月17日晚，君实生物披露了2024年度业绩预告。经财务部门初步预算，2024年实现营业收入约19.49亿元，同比增长29.71%左右；净利润亏损12.92亿元，同比收窄超4成。 尽管财务层面有好消息，投资者情绪还算冷静，1月20日，公司股价微涨1.11%。 2024年，君实生物通过提质增效的确省了不少钱。据前三季度财报，公司管理费用降低了1.7%；业绩预告提到，公司研发费用同比减少了34.24%。 但是，君实生物的业绩增长动力几乎都压在了PD-1这个竞争十分严重的品种身上，这在国内创新药企中还是比较罕见的。君实生物很早就设计了围绕PD-1开展联合治疗的用药思路，但在执行过程中一直没变过，和行业整体发展进程相比显得格格不入。 如今，PD-1赛道极致成熟，已有企业出现在“死亡名单”上。以此基本盘放在资本市场，已见熹光的君实，又好像站在了悬崖的边缘。 PD-1，难支256亿元市值 2024年，君实生物营业收入主要依靠四款商业化药品的收入增长：特瑞普利单抗注射液、阿达木单抗注射液、新冠药氢溴酸氘瑞米德韦片和昂戈瑞西单抗注射液。其中，特瑞普利单抗注射液在国内市场销售额有着大幅增长。 现有数据表明，特瑞普利单抗在今年前三季度已经分别实现营收3.07亿元、3.64亿元和3.97亿元，同比增长了56.6%、45%、79.6%，年度收入已确定超过10亿元。 但客观来说，这份成绩算不上亮眼。彼时，与公司并称“PD-1四小龙”的恒瑞医药、百济神州、信达生物，都手握国内第一批获批的PD-1产品。目前，以上产品在肝癌、肺癌、食管癌等大部分适应症都有交叉覆盖。据公开数据，2023年，恒瑞医药的PD-1销售额就达到了30亿元；百济神州的产品销售额逐年爬升，2023年就达到38.06亿元；信达生物更是在2024年预计将首次越过30亿元大关。 君实自己还是十分有信心的，鼻咽癌算是君实比较看重的一个适应症。2023年10月，君实生物将该药海外权益给了Coherus。在Coherus最新的业务规划中，每年美国约有2000例鼻咽癌新发患者，特瑞普利单抗仅美国鼻咽癌适应症的年销售峰值约为2亿美元。 可惜，海外伙伴资金周转也很艰难，其中一笔里程碑付款还要拖到今年第一季度。要说靠着分红支撑公司市值，也实在牵强。 君实还在扩充以特瑞普利单抗为核心的矩阵。预告的报告期内，特瑞普利单抗在国内的TNBC、肾癌等一线治疗的新适应症成功拿下；一线治疗黑色素瘤、联合贝伐珠单抗一线治疗晚期肝细胞癌上市申请也正在受理中。 国内市场上PD-1竞争十分激烈，目前已上市的PD-1单抗就有11款，据Insight数据库显示，处于申报上市阶段的PD-(L)1，单算III期阶段的管线也超百条，涉及了数百家企业，不乏实力强硬的荣昌生物、开拓药业、嘉和生物、宏成药业等公司，市场已经难见缝隙。 并且在国内市场，这场厮杀已经见血腥味了。1月13日，9年未上市的丽珠割掉1.6亿元终止研发旗下PD-1；誉衡生物也因该品种未达预期提计资产减值…… 单凭“老大哥”特瑞普利单抗撑着，君实能走多远尚未可知。君实方面表示：“2025年，特瑞普利单抗在国内获批的十个适应症将全部纳入国家医保目录。”想凭借医保通道放量，君实还需要时间。 创新激情，不能褪去 从财报数据来看，君实业绩增长点是从研发中省出来的。2023年，公司的研发费用从2022年的23.84亿元下降到不到20亿元。2024年更为明显：上半年研发费用为5.46亿元，与2023年相比直接砍掉了42.4%；前三季度投入8.74亿元，为2023年同期的67.59%。 大幅削减研发投入，这意味着公司要重新分配手中的业务比重了？ 君实生物执行董事、总经理兼首席执行官邹建军曾公开表示：“公司是将资源聚焦于更有潜力的研发管线，这些管线还没到大举推进临床的阶段，所以研发费用有所降低”。意思是并没有褪去创新激情，只是有的放矢。 实际上，君实生物的创新驱动力是有的，但成色谈不上好。自免明星靶点IL-17A单抗，代号JS005的项目早在2019年就已经获批临床，目前君实已经推进到临床后期。商业前景更有原研诺华的司库奇尤单抗大山，今年单只价格再降10%成功续约了医保。 还有BTLA单抗，围绕特瑞普利单抗联合疗法，早于2014年就研发立项，目前已经有两项适应症达到了III期临床阶段。目前，市场相对来说比较宽裕，只是距离变现还有段时间，公司预计tifcemalimab上市申请最快将于今年递交。 当前的君实生物最缺乏的是一个绝对的创新大单品。去年10月，公司最新上市了PCSK9单抗昂戈瑞西单抗注射液，但已经是国内第五款。在该领域，君实生物可以说是国内先吃螃蟹的企业，2017年第一个拿到临床批件后，行业都默认了这款药的领头地位，但随着安进、赛诺菲、信达生物、康方生物等其他企业的赶超，本该有大单品潜力的产品，就这么落后了。 现阶段的君实生物固然不会放弃创新，但更想调和研发创新和商业化两头并进。可以看到，君实生物已经在重新调整研发与销售的平衡点，2024年前三季度销售费用同比增加了超1亿元，用于搭建新的商业化路径。邹建军曾表示，目前公司的商业化效率明显提升。比较明显的是，2024年上半年，特瑞普利单抗的营收实现了50%同比增长。 撰稿丨苗苗 编辑丨江芸 贾亭 运营｜廿十三 插图｜视觉中国 声明：健识局原创内容，未经许可请勿转载 委员热议集采药质量；四名官员贪了2.2亿；医药妖股将被ST 2024第五届论健·年度星榜“年度营销变革之星”揭晓！ 今年流行的“人偏肺病毒”，到底是个什么毒？
