Acts as a substrate for the production of iodinated thyroid hormones thyroxine (T4) and triiodothyronine (T3) (PubMed:32025030, PubMed:17532758). The synthesis of T3 and T4 involves iodination of selected tyrosine residues of TG/thyroglobulin followed by their oxidative coupling in the thyroid follicle lumen (PubMed:32025030). Following TG re-internalization and lysosomal-mediated proteolysis, T3 and T4 are released from the polypeptide backbone leading to their secretion into the bloodstream (PubMed:32025030). One dimer produces 7 thyroid hormone molecules (PubMed:32025030).

Pilot Study of Adoptive Cell Transfer for the Treatment of Metastatic Cancer That Expresses NY-ESO-1 Using Lymphodepleting Conditioning Followed by Infusion of Anti-NY ESO-1 Murine TCR-Gene Engineered Lymphocytes

This pilot clinical trial studies the side effects of anti-ESO (cancer/test antigen) murine T-cell receptor (mTCR)-transduced autologous peripheral blood lymphocytes and combination chemotherapy with cyclophosphamide and fludarabine phosphate in treating patients with cancer that has spread to other places in the body (metastatic) and expresses the gene NY-ESO-1. Donor white blood cells that are treated in the laboratory with anti-cluster of differentiation (CD)3 may help treat metastatic cancer. Drugs used in chemotherapy, such as cyclophosphamide and fludarabine phosphate, work in different ways to stop the growth of cancer cells, either by killing the cells, by stopping them from dividing, or by stopping them from spreading. Giving more than one drug (combination chemotherapy) may kill more cancer cells. Aldesleukin may stimulate white blood cells, including natural killer cells, to kill metastatic cancer cells. Giving anti-ESO (cancer/test antigen) mTCR-transduced autologous peripheral blood lymphocytes together with combination chemotherapy and aldesleukin may kill more cancer cells.

Concurrent Benign Iodine-Avid Thymic and Liver Cysts Mimicking Metastatic Disease on 131I Whole-Body Scintigraphy.

ABSTRACT:

A 57-year-old woman received radioiodine therapy post total thyroidectomy for pT3aNxMx follicular thyroid carcinoma. Posttherapy 131I whole-body scan showed 131I concentration in the chest, mediastinum, and left upper thigh with stimulated thyroglobulin (Tg) of 89 μg/L. Subsequent radioiodine therapies showed persistent 131I accumulation in the anterior mediastinal soft tissue lesions and a hypodense segment VII liver lesion visualized on SPECT/CT, suggestive of iodine-avid metastatic disease despite the undetectable serum Tg (<1.0 μg/L) with no Tg antibody interference. Biopsy of the liver lesion revealed liver cyst, and consequent removal of the mediastinal lesions showed benign thymic cysts.

Dose-response association of diabetic kidney disease with routine clinical parameters in patients with type 2 diabetes mellitus: a systematic review and meta-analysis.

Background:

Diabetic kidney disease (DKD) is a leading cause of end-stage kidney disease and is associated with high mortality rates. The influence of routine clinical parameters on DKD onset in patients with type 2 diabetes mellitus (T2DM) remains uncertain.

Methods:

In this systematic review and meta-analysis, we searched multiple databases, including PubMed, Embase, Scopus, Web of Science, and Cochrane Library, for studies published from each database inception until January 11, 2024. We included cohort studies examining the association between DKD onset and various clinical parameters, including body mass index (BMI), hemoglobin A1c (HbA1c), systolic blood pressure (SBP), diastolic blood pressure (DBP), total cholesterol (TC), triglycerides (TG), low-density lipoprotein (LDL), high-density lipoprotein (HDL), and serum uric acid (UA). Random-effect dose-response meta-analyses utilizing one-stage and/or cubic spline models, were used to estimate correlation strength. This study is registered in PROSPERO (CRD42022326148).

Findings:

This analysis of 46 studies involving 317,502 patients found that in patients with T2DM, the risk of DKD onset increased by 3% per 1 kg/m² increase in BMI (relative risk (RR) = 1.03, confidence interval (CI) [1.01-1.04], I² = 70.07%; GRADE, moderate); a 12% increased risk of DKD onset for every 1% increase in HbA1c (RR = 1.12, CI [1.07-1.17], I² = 94.94%; GRADE, moderate); a 6% increased risk of DKD onset for every 5 mmHg increase in SBP (RR = 1.06. CI [1.03-1.09], I² = 85.41%; GRADE, moderate); a 2% increased risk of DKD onset per 10 mg/dL increase in TG (RR = 1.02, CI [1.01-1.03], I² = 78.45%; GRADE, low); an 6% decreased risk of DKD onset per 10 mg/dL increase in HDL (RR = 0.94, CI [0.92-0.96], I² = 0.33%; GRADE, high), and a 11% increased risk for each 1 mg/dL increase in UA (RR = 1.11, CI [1.05-1.17], I² = 79.46%; GRADE, moderate). Subgroup analysis revealed a likely higher risk association of clinical parameters (BMI, HbA1c, LDL, and UA) in patients with T2DM for less than 10 years.

Interpretation:

BMI, HbA1c, SBP, TG, HDL and UA are potential predictors of DKD onset in patients with T2DM. Given high heterogeneity between included studies, our findings should be interpreted with caution, but they suggest monitoring of these clinical parameters to identify individuals who may be at risk of developing DKD.

Is Every Thyroid Antibody a Bad Sign?: The Complex Relationship of Antithyroid Antibodies and Obsessive-compulsive Symptoms.

Objective:

: Several immunological factors are emphasized in the etiology of autoimmune thyroid diseases and obsessivecompulsive disorder. Obsessive-compulsive symptoms (OCS) are commonly seen in patients with autoimmune thyroid diseases. This study aims to evaluate the relationship between OCS and antithyroid antibodies.

Methods:

: The study included 145 patients with Hashimoto thyroiditis or Graves' disease and 42 healthy controls. Thyroid function tests and serum thyroid autobody levels (anti-thyroglobulin [TG], anti-thyroid peroxidase [TPO], and anti-thyroid stimulating hormone [TSH]) of the patients were measured. The socio-demographic data and OCS of the participants were evaluated with Dimensional OCS (DOCS).

Results:

: DOCS scores were higher in patients than in the control group. There was not found a significant relationship between free T3, free T4, and TSH levels and DOCS scores. Anti-TG positivity in females was associated with lower DOCS scores. Anti-TPO positivity in males had a positive correlation with DOCS scores. There was no correlation between sex and the presence of anti-TSH in terms of OCS severity. Univariate analysis found the highest OCS scores in anti-TPO positive, anti-TG, and anti-TSH negative patients. The group with the lowest OCS scores was found to be anti-TG positive, anti-TPO, and anti-TSH negative patients.

Conclusion:

: OCS severity could be affected by different thyroid autoantibody profiles in patients with autoimmune thyroid diseases. While anti-TG serves a protective role against OCS in females, the presence of anti-TPO may worsen the OCS in men. Additionally, the co-existence of different antithyroid antibodies may affect the severity of OCS differently according to sex.

发现骨脑轴新机制！

*仅供医学专业人士阅读参考如果直到现在，你还单纯地认为骨头的作用是支撑身体的一根梁，那就大错特错了。最近的研究发现，骨骼除了支撑身体、保护软组织器官和促进运动等作用外，还可能是一个内分泌器官。虽然骨骼看上去与柔软的内分泌器官并不沾边，但是它确实可以分泌多种细胞因子，比如骨钙素、成纤维细胞生长因子23、骨保护素等，参与骨或非骨器官的稳态的调剂。随着年龄增长，内分泌稳态发生变化，各种神经内激素水平变化影响着大脑神经元的衰老和死亡。与此同时，来自其他器官的激素水平变化也有可能对大脑产生影响。近日，南京大学医学院附属鼓楼医院的郭保生、蒋青与石云团队，发现了骨细胞分泌的硬化蛋白（Sclerostin）与大脑衰老和β-淀粉样蛋白（Aβ）之间的关系。他们发现，这种骨硬化蛋白可以穿过老龄小鼠的血脑屏障，高水平的骨硬化蛋白导致Wnt-β-catenin通路失调，加速Aβ的产生和积累，损害老龄小鼠的突触可塑性和记忆力。研究发表在《自然·代谢》上。骨硬化蛋白主要由成熟的骨细胞分泌，在骨代谢过程中起到调节作用，也是Wnt-β-catenin通路的拮抗剂。骨硬化蛋白水平升高常见于骨质疏松患者，其他疾病，比如2型糖尿病、肝病、肾脏疾病等患者也可以观察到血清骨硬化蛋白水平较高，还会随年龄增长而逐渐升高，因此研究人员猜测，骨硬化蛋白失调可能对骨组织之外的器官产生影响，甚至有可能影响到大脑功能。Wnt-β-catenin通路在成人神经系统中也发挥着重要作用，与突触可塑性和记忆有关，在衰老和AD进展期间，也可以观察到Wnt-β-catenin通路活性减弱，激活Wnt-β-catenin通路可以改善病理相关突触功能受损。在本次研究中，研究人员将22月龄和12月龄小鼠进行对比，发现骨硬化蛋白可以穿过22月龄小鼠的血脑屏障，引起大脑中的骨硬化蛋白水平显著增加，导致Wnt-β-catenin通路失调。骨硬化蛋白在衰老过程中介导大脑Wnt信号信号失调通过转基因技术构建骨硬化蛋白水平升高的小鼠模型（TG），可以发现，与野生型小鼠相比，12月龄的TG小鼠记忆和空间探索能力下降，皮层和海马区域棘突数量减少，18月龄时就表现出严重的突触损伤。接下来，研究人员构建了骨硬化蛋白缺失的小鼠模型（cKO），18月龄的cKO小鼠表现出比野生型小鼠更好的记忆和空间探索能力；同样，降低老龄小鼠的骨硬化蛋白水平，也可以改善小鼠的认知和突触功能。高水平骨硬化蛋白影响小鼠认知和突触功能但是，骨硬化蛋白对AD进展的影响并不是通过破坏血脑屏障实现的，12月龄小鼠的血脑屏障并没有被明显破坏的痕迹。研究人员的下一个备选的猜测是骨硬化蛋白通过影响Aβ的产生途径参与AD进展。果然，研究人员发现TG小鼠皮质和海马中β-catenin水平较低，BACE1水平较高，骨硬化蛋白通过抑制Wnt-β-catenin通路促进BACE1表达和Aβ形成。在AD小鼠模型中，研究人员通过尾静脉注射上调骨硬化蛋白水平，注射2个月后，小鼠血清和脑组织中的骨硬化蛋白水平升高，皮质和海马均受到影响，Aβ40和Aβ42的浓度增加，Aβ沉积也明显增多，骨硬化蛋白通过促进Aβ产生加快了AD小鼠的疾病进展。抗骨硬化蛋白给药可以改善老年小鼠和高骨硬化蛋白水平下AD小鼠的记忆受损，还可以减轻小鼠的骨密度下降和骨微结构恶化。高水平骨硬化蛋白与老年人群和AD患者的认知功能障碍有关在老年人群中，血清骨硬化蛋白水平与认知功能评分呈负相关，血清骨特异性碱性磷酸酶水平也根据根据认知功能评分不同而有明显区分，说明高水平血清骨硬化蛋白水平与老年人骨代谢紊乱和认知功能受损有关。同样，在AD患者中有观察到血清骨硬化蛋白水平与认知功能评分呈负相关，并且与血清Aβ水平呈正相关。骨代谢紊乱和认知功能受损都是常见的衰老相关器官功能失调。在这项研究中，研究人员揭示了骨硬化蛋白对衰老和AD进展过程中认知功能的影响。骨细胞来源的硬化蛋白水平异常升高使大脑Wnt-β-catenin通路失调，加速病理条件下的认知功能下降。研究为骨细胞生物学与神经退行性疾病之间的联系提供了坚实的依据，对预防或治疗AD及其他年龄相关疾病具有广泛影响。参考文献：Shi T, Shen S, Shi Y, et al. 里程碑！首款NASH新药获FDA加速批准上市

3月14日，FDA官网显示，Madrigal Pharmaceuticals开发的Resmetirom（商品名：Rezdiffra）获加速批准上市，用于治疗伴有肝纤维化的非酒精性脂肪性肝炎（NASH，现称为代谢功能障碍相关脂肪性肝炎（MASH））患者。这是FDA批准的首个治疗NASH的创新药物。Resmetirom是一款甲状腺激素受体（THR）-β口服选择性激动剂。THR-β在人体肝脏中高表达，能够调节脂代谢，降低LDL-C、甘油三酯和致动脉粥样硬化性脂蛋白。此外，THR-β还可以通过促进脂肪酸的分解和刺激线粒体的生物发生来减少脂肪毒性并改善肝功能，进而减少肝脏脂肪。因此，THR-β激动剂具备调控多种肝脏代谢通路来治疗NASH的潜力。目前，Madrigal针对Resmetirom已经开展了4项III期临床试验，分别为MAESTRO-NASH、MAESTRO-NAFLD-1、MAESTRO-NAFLD-OLE以及 MAESTRO-NASH-OUTCOMES研究。MAESTRO-NASH研究是一项多中心、随机、双盲、安慰剂对照的III期临床试验，启动于2019年3月，评估了Resmetirom用于肝活检确诊为NASH患者的疗效与安全性。该研究H部分招募1000多例NASH患者，至少一半为F3（晚期纤维化），其余为F2或F1B（中度纤维化），少数为早期F1患者，这些患者按1:1:1分为三组，分别接受Resmetirom 80mg、100mg或安慰剂治疗。双重主要替代终点是治疗52周肝穿的组织学结果，患者NASH消退（NAS评分下降超过2分）且纤维化没有恶化，或NASH没有恶化且纤维化改善≥1级，实现二者中任意一个均可视为达到主要终点；关键次要终点为治疗24周时LDL-C降低水平。研究结果显示，在NASH缓解且纤维化不恶化的主要终点上，高剂量组、低剂量组和安慰剂组的发生率分别为30%、26%和10%；在纤维化改善≥1级且NASH不恶化的主要终点上，三个组的发生率为26%、24%和14%；在次要终点LDL-C降幅上，三个组分别为-16%、-12%和1%。来源：Madrigal Pharmaceuticals官网参加MAESTRO-NASH研究的患者（约1750例）在52周治疗期后继续接受长达54个月的治疗，以检测活检进展为肝硬化（52周和54个月）、肝硬化失代偿期以及全因死亡率。针对NAFLD患者的多中心、随机、安慰剂对照的III期MAESTRO-NAFLD-1研究已于2019年12月启动，患者按1:1:1:1分为四组，分别接受Resmetirom 80mg或100mg、安慰剂（双盲）、Resmetirom 100mg（开放标签），为期52周。该研究主要终点为安全性，关键次要终点包括LDL-C、载脂蛋白B（apoB）、甘油三酯（TG）、肝脏脂肪含量（MRI-PDFF）等指标的降低水平。结果显示，双盲人群中Resmetirom安全性良好。80mg以及100mg剂量组均达到关键次要终点，LDL-C、apoB、TG、MRI-PDFF以及CAP值均显著降低（p<0.0001）。NASH是非酒精性脂肪性肝病（NAFLD）的一种严重形式，其特征在于脂肪在肝脏中过度堆积，对肝细胞产生应激和损伤，引起炎症和纤维化，进而发展为肝硬化、肝功能衰竭、癌症甚至死亡。对整个医药行业来说，NASH一直是个难题，其需求远未得到满足，目前尚无治愈或特异性的治疗方法。据估计，到2026年，NASH市场将达到183亿美元。Copyright © 2024 PHARMCUBE. 2023年美国甲状腺协会（ATA）年会热点荟萃

2023年9月27日-10月1日，美国甲状腺协会（ATA）年会于美国华盛顿举办了百年庆典暨2023年年会，世界各地甲状腺领域的临床医生和研究专家同聚一堂，共话甲状腺疾病最新的临床和基础研究进展，为提升甲状腺疾病诊疗水平添砖加瓦。借着此次年会的平台，THYROID的主编Anna M.
