A vascular malformation of developmental origin characterized pathologically by ectasia of superficial dermal capillaries, and clinically by persistent macular erythema. In the past, port wine stains have frequently been termed capillary hemangiomas, which they are not; unfortunately this confusing practice persists: HEMANGIOMA, CAPILLARY is neoplastic, a port-wine stain is non-neoplastic. Port-wine stains vary in color from fairly pale pink to deep red or purple and in size from a few millimeters to many centimeters in diameter. The face is the most frequently affected site and they are most often unilateral. (From Rook et al., Textbook of Dermatology, 5th ed, p483)

A Multi-Center, Randomized, Double-Blind, Vehicle-Controlled, Sequential Group Comparison Study to Evaluate the Safety and Efficacy of Light Dose (Fluence) in Subjects With Port-wine Birthmark of the Face and/or Neck Treated With Hemoporfin + Green Light Photodynamic Therapy

This is a multi-center, randomized, double-blind, vehicle-controlled, and sequential group Phase 2 study. Eligible subjects aged 18 to 65 years old with PWB of face and/or neck area (except subjects for Stage One) will receive Hemoporfin PDT or vehicle PDT in 8-week cycles at fixed drug dose (5 mg/kg) and different light fluence.

Tolerability of 532 nm Laser Treatment of Port Wine Stains

The goal of this study is to learn about treatment of port wine birthmarks treated with an FDA-approved 532 nm laser.
The main questions it aims to answer are:
How well are the treatments tolerated?
Are there differences in tolerability of the treatment when a single high fluence laser pulse is used (the standard treatment) versus using multiple low fluence pulses?
Are there differences in results when using a single high fluence pulse versus multiple low fluence pulses?
type of study: Clinical Trial
Participants will undergo 3 monthly laser treatments with the 532 nm DermaV laser. Part of their birthmark will be treated with the standard single-pulse high fluence approach, and other parts will be treated with the multiple-pulse low fluence approach.

Recurrent hematuria involving urinary system with Klippel-Trenaunay syndrome: A case report.

RATIONALE:

Klippel-Trenaunay syndrome (KTS) is a rare congenital venous malformation, it had been found to be caused by mutations of the phosphatidylinositol 4, 5-diphosphate 3-kinase catalytic subunit alpha (PIK3CA) gene. Currently KTS is defined as a triad of skin wine pigmented spots, varicose veins and malformations of the lower extremities, and hypertrophy of bone and soft tissue, involving urinary system up to 6% to 30%. When the urinary system is involved, KTS is often presented as painless massive gross hematuria.

PATIENT CONCERNS:

This article describes a woman who was hospitalized with painless massive gross hematuria. Physical examination revealed significant hypertrophy of the right lower limb with varicose veins, port-wine stains in the skin, and right perineal hemangiomatous changes with swelling. The patient was admitted to hospital 4 times for repeated hematuria and infection.

DIAGNOSES:

By physical examination, CT urography, ureteroscopy and cystoscopy, the patient was diagnosed to have Klippel-Trenaunay syndrome, involving the urinary system.

INTERVENTIONS:

The patient hematuria improved after multiple indwelling D-J tubes and anti-inflammatory treatment.

OUTCOMES:

The final symptoms of hematuria improved significantly, follow-up so far has not recurred.

LESSONS:

This case presents the possibility of painless gross hematuria with KTS. Most of patients can be improved by conservative treatment. Cystoscopic laser therapy is the preferred treatment for poor bleeding control. Cystectomy and nephrectomy should be considered when life-threatening.

Anthropogenic threats to the reintroduced population of Oriental storks in Japan: analysis of the causes of injury and death of 153 storks from 2005 to 2021.

After the native population of Oriental stork (Ciconia boyciana) in Japan disappeared, a reintroduction project was implemented in 2005. All released storks and most wild-fledged storks were individually identified using colored rings on their legs. The size of the reintroduced population reached 256 by the end of 2021. In this study, we investigated the causes of stork injury and death in wild populations to clarify important medical issues for successful reintroduction. During the survey, 153 of 412 (78 released and 334 fledged storks) storks were injured or died between 2005 and 2021. At least 49.7% of the injuries and deaths were directly caused by human activities. Entanglement with pest control measures (such as bird- and beast-proofing nets) and accident with electrical and telecommunication equipment are two major causes of injury and death of reintroduced Oriental storks in Japan. It is important to recognize that these anthropogenic threats have a significant impact on the establishment and maintenance of the reintroduced populations of Oriental storks in Japan. Therefore, it is necessary to implement countermeasures against these threats to establish human and Oriental storks coexistence, which is a major goal of reintroduction.
