Continuous positive airway pressure (CPAP) is the mainstay of treatment recommended for preterm infants with respiratory distress syndrome in all resource contexts. Many of the available bubble CPAP machines in low- and middle-income contexts mix flows of air and oxygen. These are adjusted by two separate rotameters (flowmeters) and estimate FiO2 delivery. Medical decisions regarding interventions to sick neonates such as need for intubation or surfactant administration are in large part dependant on a reliable estimation of FiO2-delivery. The aim of this study is to evaluate the accuracy of FiO2 delivered by bubble-CPAP machines with separate oxygen and air rotameters in patients at a neonatal intensive care unit (NICU) in Hanoi.

A Single-arm, Prospective Feasibility Study of Placement and Surfactant Administration Through a Preterm Size Laryngeal Mask Airway in Very Low Birth Weight Neonates, with Respiratory Distress Syndrome, At a Level III Neonatal Intensive Care Unit in Hanoi, Vietnam

Surfactant administration via a supraglottic airway device or laryngeal mask airway (SALSA) is a minimally invasive method of instilling surfactant in the trachea during spontaneous breathing and after applying nasal continuous positive airway pressure (nCPAP). However, the procedure has been limited from use in very low birth weight neonates, due to lack of preterm size LMAs, which are now emerging on the market.
The goal of this study is to see if investigators can successfully use a new, smaller laryngeal mask airway (LMA) to place and give surfactant to premature babies with respiratory distress syndrome (RDS) who weigh between 750g and 1500g at birth.
The main objectives of the study are to:
* Check the placement of the new, smaller LMA: This includes evaluating the airway, how long it takes to place the LMA, how many attempts are needed, and the baby's stability during the process.
* Evaluate the administration of surfactant using the new LMA: Investigators will look at how the baby responds clinically, any changes in oxygen needs, how many doses of surfactant are required, the level of respiratory support needed, and whether intubation or mechanical ventilation is necessary.
* Ensure the safety of using the new LMA and administering surfactant: Investigators will monitor the baby's stability during the procedure and watch for any adverse events.
* Assess pain during the procedure: Investigators will evaluate pain levels using a pain scale based on video reviews.
Above objectives of feasibility are to be assessed before proceeding to a large randomize clinical trial assessing effectiveness and safety.

The Impact of PEEP-guided Electrical Impedance Tomography on Oxygenation and Pulmonary Mechanics in Moderate-to-severe ARDS

Acute respiratory distress syndrome (ARDS) in its moderate to severe forms is associated with high mortality. Mechanical ventilation (MV) remains the cornerstone of ARDS management but carries a significant risk of ventilator-induced lung injury (VILI). Positive end-expiratory pressure (PEEP), a fundamental component of MV, is widely utilized in clinical practice; however, optimal PEEP selection for patients with moderate to severe ARDS remains a complex and unresolved challenge. Electrical impedance tomography (EIT), a bedside imaging modality that evaluates regional ventilation distribution, offers a means of individualizing PEEP settings in mechanically ventilated patients. By balancing the competing risks of alveolar overdistension and collapse, EIT facilitates precision in PEEP titration. This study compares the impact of EIT-guided PEEP selection versus the conventional low FiO2-PEEP table on blood oxygenation and pulmonary mechanics.

Association of testicular histopathology with sperm retrieval success rates in men with idiopathic non-obstructive azoospermia

INTRODUCTIONInfertility is a major public health issue, with male factors alone contributing to 20-30% of cases. Non-obstructive azoospermia (NOA) is the most severe form, and although techniques like microdissection testicular sperm extraction (mTESE) offer hope, it remains challenging due to its uncertain causes. This study investigates the correlation between testicular histopathology and clinical parameters to enhance sperm retrieval (SR) prediction.MATERIALS AND METHODSWe reviewed 57 azoospermic men from Hanoi Medical University Hospital, recruited between January 2021 and September 2023. Inclusion criteria were confirmed azoospermia and exclusion of known NOA causes. All underwent mTESE with testicular biopsies classified into four histopathological patterns.RESULTSThe patients' mean age was 31.75 ± 5.19 years. SR was successful in 19.3% (11/57). Higher follicle-stimulating hormone (FSH) levels were noted in SR-positive men (p = 0.02). Sertoli cell-only syndrome (SCOS) was the most common pattern. While we found a significant difference in SR rate between testicular histopathology, multivariate analysis showed no strong association. However, FSH levels were predictive of histopathology patterns.CONCLUSIONSIdiopathic NOA (iNOA) represents over 60% of azoospermia cases. mTESE remains the gold standard, and FSH levels may help predict testicular histopathology patterns and improving patients prognosis of SR outcomes.

Accuracy of total knee arthroplasty using hospital-based 3D printed patient-specific instrumentation: A radiology research

BackgroundPatient-specific instruments (PSIs) were reported to improve implant position and rotation accuracy in total knee arthroplasty (TKA), among other benefits. Most PSIs used in TKA were commercial products from implant manufacturers, which could be time-consuming and could potentially affect accuracy as they relied on engineer's decision. This study aimed to present the radiological outcomes of PSI produced by the 3D printing facility at our hospital, with direct involvement of the surgeon in its design.MethodsThis prospective study enrolled 45 patients who underwent mechanically aligned TKA. Preoperative whole-limb 3D-CT scans were obtained to create the sizing plan. The surgeon identified the position of the cut plane and guide pin locations on software, which were then forwarded to engineers for further processing. Subsequently, the PSI was fabricated using in-hospital 3D resin printers. During surgery, after removing all remaining cartilage, the PSI was utilized on the bone surfaces to assist surgeons in drilling pinhole positions for the manufacturer's cutting instruments. Post-surgery, CT scans were acquired and overlaid onto the 3D model of the implant to compare to the plan.ResultsOn average, it took three days to prepare for the surgery using PSI. 100 % of patients had implant sizes that matched the preconception template. The mean postoperative hip-knee angle was 178.1° ± 1.34°. The mean femoral component rotation angle relative to the surgical transepicondylar axis (sTEA) and the mean rotation mismatch between the femur and tibial component were 0.99° ± 0.84° and 0.91° ± 1.61°, respectively.ConclusionsPreliminary radiological results demonstrated that in-hospital PSI was a useful technique for improving the accuracy of implant positioning in TKA, especially in controlling the rotational alignment.

Body image research among sexual minority men in the Mekong region: A scoping review

Body image among sexual minority men (SMM) has received increasing attention. However, the current literature has primarily focused on white SMM and body image and physical appearance concerns among SMM. In response to a call for more nuance in understanding how SMM perceive and make sense of their body image in a broader sociocultural context, we conducted a scoping review to examine the extent and nature of body image research among SMM in the Mekong region. We searched CINAHL, MEDLINE, Scopus, PsycInfo-1806 (Ovid), ProQuest Central, Social Science Premium Collection, and Web of Science and identified 25 studies that met the inclusion criteria. Overall, the findings suggest that body image research in the Mekong region is limited in the range of topics, study populations, and methods, with quantitative studies on preferences for masculine physical traits among young and well-educated Chinese SMM dominating. Furthermore, the current literature is primarily influenced by Western scholarship, which predominantly uses Western measurement and theories. We encourage scholars from other countries in the region, including Vietnam, Laos, Cambodia, Myanmar, and Thailand, to engage sociocultural approaches in research on the body image of SMM.

Lululab launching AI beauty research centre with Yonsei Cancer Center

South Korean AI beauty service startup Lululab has tied up with Yonsei Cancer Center's Scar Laser and Plastic Surgery Center to take their research partnership on the use of AI to treat skin diseases to a new level. The organisations recently signed a memorandum of understanding to set up a Skin AI Beauty Research Center. According to a press statement, the research centre will serve as Lululab's bedrock of AI solutions for diagnosing skin diseases optimised for hospitals. It plans to market its technology to its overseas clients, which make up 90% of its client base. The facility will also be utilising Lululab's own AI-powered skin analysis tool for conducting research on various skin diseases, such as scar, atopy, and acne, and on the development of skin disease solutions. Lululab will also make use of its own AI-based customer relationship management system, which links skin analysis reports and treatment information to medical devices. WHY IT MATTERS Through the research centre, Lee Joo-hee, its upcoming head, said a predictive model to determine the potential effects of scarring and cosmetic treatments will be developed. He expects the technology to be applied to clinical practice soon. THE LARGER CONTEXT The latest MOU builds on Lululab's existing partnership with Yonsei University Severance Hospital where they jointly worked on a skin diseases database and AI-based tools for analysing skin diseases, including atopy, psoriasis, and vitiligo. Some of those AI tools are said to be ready for commercialisation. Outside South Korea, Lululab has another research partnership with Hanoi Medical University in Vietnam to co-develop similar AI-based skin disease analysis solutions, enabling the precise diagnosis of various skin diseases in the country.
