Le cathéter veineux central á insertion périphérique, souvent désigné par l'acronyme anglais << PICC Line >> et les chambres implantables (PAC) sont deux types de cathéters centraux permettant l'administration de médicaments et les prélévements sanguins. Ces deux accés étant trés controversés (complications infectieuses et thrombotiques), il semble intéressant d'évaluer leur coût de pose et de les corréler aux ressources allouées par l'Assurance maladie.L'évaluation prospective a porté sur l'observation de poses de PICC Lines et de PAC durant lesquelles ont été recueillis le matériel et les médicaments utilisés (coûts directs) ainsi que le temps d'utilization de la salle et le personnel intervenant (coûts indirects).Le coût global d'une pose de PICC Line est de 220,2 euros et celui d'une chambre implantable de 286,6 euros en radiologie interventionnelle et de 666,3 euros au bloc opératoire.Seule une pose de Picc Line en soins externes peut être valorisée. Le montant du remboursement alloué par l'Assurance maladie est alors de 110,4 euros. L'établissement réalise donc des poses sans bénéfice. Cependant, l'utilization des PICC Lines offre aux patients un accés rapide á une voie veineuse centrale et donc une prise en charge thérapeutique optimale, principale mission des établissements publics de santé. Quant au PAC, ils ne rapportent que si la pose est effectuée lors d'une hospitalisation en ambulatoire. Sa pose en radiologie semble plus rentable puisque les coûts indirects sont beaucoup plus modérés.Peripherally Inserted Central Catheter or PICC Line and implanted s.c. ports are two types of central catheters allowing drug administration and blood samplings. These two devices are very controversial (because of infectious and thrombotic complications), it seemed interesting to estimate their cost of implantation and to correlate them with the reimbursement by the Health Insurance.Direct (material and drugs) and indirect (use of the room and staff) costs were prospectively evaluated for PICC Lines and implanted s.c. ports.The global costs of the implantation of a PICC Line and of an implanted s.c. port in the interventional radiol. room and in the operating room were resp. evaluated at 220.2 euro, 286.6 euro and 666.3 euro.Only a PICC Line in outpatients can be reimbursed by the health insurance; which amounts to 110.4 euro. The establishment therefore loses money with every implantation. However, PICC Lines offer to the patients a fast access to a central venous way and thus an optimal therapeutic care, fulfilling one of the main missions of the public health institutions. Implanted s.c. ports are economically worth being implanted only in ambulatory inpatients. Its implantation in radiol. seemed more profitable because the indirect costs were much more moderate.
