A prodromal phase of infection with the human immunodeficiency virus (HIV). Laboratory criteria separating AIDS-related complex (ARC) from AIDS include elevated or hyperactive B-cell humoral immune responses, compared to depressed or normal antibody reactivity in AIDS; follicular or mixed hyperplasia in ARC lymph nodes, leading to lymphocyte degeneration and depletion more typical of AIDS; evolving succession of histopathological lesions such as localization of Kaposi's sarcoma, signaling the transition to the full-blown AIDS.

Incidence and Associated Risk Factors of Augmented Renal Clearance (ARC) in Critically Ill Trauma Patients of 50 Years Old and Above

A Multi-national Phase IIIb, Double-blind, Placebo-controlled Trial to Determine the Safety and Efficacy of STALORAL® Birch 300 IR in Children and Adolescents 5 to 17 yo With Birch Pollen-induced Allergic Rhinoconjunctivitis w/o Asthma

Allergic rhinoconjunctivitis due to birch pollen is a seasonal problem which manifests as a combination of nasal symptoms (such as congestion, runny nose, sneezing, itching of the nose) and ocular symptoms (such as red, itchy and watery eyes). For several birch-allergic patients, allergic rhinoconjunctivitis occurs with an oral allergy syndrome.
The purpose of this study is to demonstrate the safety and efficacy of the study drug (STALORAL Birch 300 IR) in children and adolescents with birch pollen-induced allergic rhinoconjunctivitis, with or without asthma, when treated before and during the pollen season.
Approximately 699 children will participate in this study. The study will be conducted worldwide in approximately 100 medical sites in about 14 countries. The total duration of the study will be approximately 20 months.

Quantification of meropenem in serum and cerebrospinal fluid in children with bacterial meningitis with augmented renal clearance by UPLC-MS/MS.

Meropenem is an ultrabroad-spectrum antimicrobial agent that is often recommended for the treatment of bacterial meningitis (BM) in children. However, a subtherapeutic phenomenon occurred in BM children complicated with augmented renal clearance (ARC) at the recommended dose of meropenem. To support its pharmacokinetics, a sensitive, fast and robust ultra-liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method was developed to measure meropenem concentrations in serum and cerebrospinal fluid (CSF). The method involved protein precipitation, and samples were diluted with a large proportion of water to eliminate solvent effects. The separation of samples was performed on a Waters Acquity™ BEH C₁₈ column (2.1 × 50 mm i.d., 1.7 μm) with a gradient profile. The mobile phases were formic acid-water (1:1000, v/v) and acetonitrile. The linear range was good, with a concentration range of 0.100-100 μg/mL for serum and 0.0400-20.0 μg/mL for CSF. The intra-day and inter-day precisions were less than 8.0%, and the intra-day and inter-day accuracies varied -6.6% from 6.5% for the both serum and CSF. The selectivity, carry-over, dilution integrity, matrix effect, recovery and stability were validated according to international guidelines. The developed UPLC-MS/MS method successfully determined the meropenem concentrations in the serum and CSF of children with BM complicated with ARC. The results indicated that under the recommended dosing regimen (40 mg/kg every 8 h), the time to reach the effective treatment target of 50%T > MIC was only approximately 3 h and lower CSF concentrations of meropenem were observed in children with BM with ARC.

A 91-year-old man was admitted with vomiting and abdominal pain. He had had COVID-19 pneumonia a month before and the treatment had consisted of remdesivir, dexamethasone and baricitinib. CT scans showed pneumatosis intestinalis. His respiratory condition rapidly deteriorated and chest CT scans showed ground-glass opacity and Strongyloides stercoralis was identified in the sputum, making a diagnosis of hyperinfection syndrome associated acute respiratory distress syndrome. Treatment of ivermectin was not achieved in time and he died of multiple organ failure. S. stercoralis is a soil-transmitted helminth endemic to tropical and subtropical areas. Immunosuppressive conditions can cause hyperinfection syndrome and life-threatening conditions. Our case highlights the importance of assessing for untreated chronic strongyloidiasis in COVID-19 patients requiring steroid treatment in endemic areas.

Clinical Outcomes of Protocolized Antibiotic Dosing in Critically Ill Surgical Patients With Bacteremia and Augmented Renal Clearance Compared With Critically Ill Surgical Patients Without Augmented Renal Clearance.

Background: Augmented renal clearance (ARC) is a phenomenon observed in critically ill patients, leading to supraphysiologic drug clearance and concern for suboptimal antibiotic concentrations. The purpose of this study was to compare the clinical outcomes of our institutional protocolized antibiotic dosing regimen in critically ill patients with bacteremia and ARC compared with critically ill patients without ARC. Patients and Methods: We performed a retrospective study comparing the efficacy of an institutional protocolized antibiotic dosing regimen in critically ill patients with bacteremia and ARC compared with critically ill patients without ARC. The primary end point was in-hospital mortality. Secondary outcomes were intensive care unit (ICU) mortality, days requiring mechanical ventilation, ICU length of stay (LOS), hospital LOS, development of drug resistance to index antibiotic agent, and documented clearance of blood cultures within 72 hours. Results: There were 75 patients included in this study. Twenty percent of patients in the ARC group died in the hospital versus 31% in the non-ARC group (p = 0.26). The results for the ARC group versus the non-ARC group for the secondary outcomes of ICU mortality (20% vs. 26%; p = 0.56), ICU LOS (14.7 days vs. 7 days; p = 0.07), hospital LOS (28.3 days vs. 21.6 days; p = 0.03), days requiring mechanical ventilation (14 days vs. 12 days; p = 0.49), duration of antibiotic therapy (7.5 days vs. 9.0 days; p = 0.39), documented clearance of blood cultures within 72 hours (41% vs. 33%; p = 0.56), and the development of drug resistance to the index antibiotic agent (0% vs. 0%; p > 0.99) were also calculated. Conclusions: Among critically ill patients with bacteremia and ARC, there was no difference in in-hospital mortality compared with critically ill patients without ARC. There was a difference in hospital LOS, with a shorter duration of stay for the non-ARC group. There was no development of multi-drug-resistant organisms in either group.

▎药明康德内容团队报道2023年7月，生物医药行业资深科学家朱梅英博士在职业生涯上再次转身，加入了专注于提供生物偶联药物端到端CRDMO服务的公司——药明合联（WuXi XDC），并担任首席技术官（CTO）这一重要职位，负责药明合联偶联技术平台的不断创新和提升，打造全球领先的偶联工艺平台，赋能全球生物偶联药行业。朱梅英博士曾先后在多家生物医药公司工作，领导并直接参与了超过25个生物药的1~3期临床开发。自2009年以来，她一直专注于新型ADC药物的研发，其中有5个ADC项目获得了美国FDA的临床批件。此外，她还主持并领导了中国首个本土研发的ADC药物上市申报的CMC工作。她在ADC领域有十五年的工作经历，积累了大量从早期发现、工艺开发、临床生产到商业化生产全方位的深度经验。“过去28年，我亲身参与并见证了单抗、融合蛋白、双特异性抗体和抗体偶联药物（ADC）等生物药的发展历程，并在该领域积累和沉淀了丰富的经验。展望自己职业生涯的新未来，我希望自己能够站在一个更高、更宽的平台，充分利用自己学到的科学知识和在药企中积累的经验帮助更多的生物医药公司，开发出更多造福病患的创新疗法。而药明合联正是专注于为生物医药公司提供生物偶联药物的赋能平台，这和我的愿望非常一致。”谈及这次职业生涯的转变，朱梅英博士如是说道。图片来源：药明合联提供公开资料显示，药明合联成立于2021年，专注于为包括ADC在内的生物偶联药物（XDC）提供端到端的服务，服务范围涵盖抗体及其它蛋白中间体、毒素连接子、生物偶联药物原液及制剂的工艺开发和生产。根据公开资料显示，截至2023年6月底，药明合联已承接了110个ADC综合项目（包括抗体和ADC工艺开发和生产）。当前，XDC已成为新药研发领域的前沿方向之一。ADC之外，还有哪些XDC值得期待？作为一种新分子类型药物，XDC当前面临的研发和生产挑战都有哪些？药明合联的技术和平台又有望解决其中的哪些难题？带着这些问题，我们采访了药明合联的首席技术官朱梅英博士。医药观澜：首先祝贺您履新药明合联首席技术官这一重要职位。您是ADC领域的资深专家，从ADC发现到商业化都有很多经验，对于这次职场身份的转换，您是如何思考的？朱梅英博士：经过近30年工作，我也在思考接下来的职业生涯规划。展望职业生涯的未来，我希望可以利用自己在生物药领域积累的丰富经验，从“平台赋能者”角度来帮助更多的公司实现创新药开发的梦想，这正是药明合联作为全球新药研发赋能者的目标，所以就选择加入了药明合联。在药明合联CTO这个职位上，我将负责公司偶联技术平台的不断创新和提升，打造全球领先的偶联工艺平台，并领导药明合联在海外的CMC管理团队，保障客户的项目在保质保量的前提下，能够以最快的速度进入临床或者商业化申报，为合作伙伴的新药研发保驾护航。医药观澜：药明合联的主要业务领域是包括ADC在内的生物偶联物（XDC）。在您看来，XDC当前的研发现状是怎样的？它们有望解决疾病治疗中的哪些难题？朱梅英博士：XDC中的“X”既代表着未知，也代表着无限的可能。随着技术的发展和创新，XDC的边界也在被不断拓展。通过对传统意义上的ADC药物进行靶点、抗体创新，毒素连接子创新，以及偶联技术创新，目前已经涌现出来许多新型的偶联药物，包括抗体放射型同位素偶联物（ARC）、抗体-寡核苷酸偶联物（AOC，包括抗体-siRNA偶联物）、抗病毒偶联药物（AVC）、多肽药物偶联物（PDC）等。现阶段，XDC类药物仍以ADC为主。目前ADC主要聚焦的适应症是癌症，而前述的ARC、AOC、PDC等新型药物的出现拓展了XDC的适应症范围。癌症之外，这类药物还有望给心血管、糖尿病、自身免疫性疾病、罕见病等患者带来治疗选择。医药观澜：您认为，当前XDC在开发和生产中面临的主要挑战有哪些？药明合联的技术和平台有助于解决其中的哪些挑战？朱梅英博士：XDC产品（包括ADC）是大分子和小分子组合而成，涉及非常多的学科以及各种不同的技术背景。一个主要的挑战是工艺方面，包括抗体工艺及小分子工艺遇到的挑战，偶联工艺本身的复杂性和敏感性带来的挑战，以及偶联物的不稳定性所引发的药物的有效性及安全性的挑战，这些都大大增加了XDC产品工艺开发的难度及工作量，对技术人员的技能和经验要求更高。另外，由于高活、高毒的特性，ADC的生产对厂房设备设施有着非常严格的要求，同时对员工的培训防护也有特别的要求，一次性投入建设生产抗体、小分子、偶联物、制剂的厂房不仅成本高，而且风险大。对目前XDC行业用到的绝大部分偶联技术，药明合联的技术团队都研究并优化过，而且把这些技术和经验平台化，为合作伙伴提供服务，这就从技术层面上减少了从头开发所需要的大量时间，提高了开发效率和成功率。同时，药明合联可以提供一套完整的从新药发现、临床前研究、CMC开发生产（包括抗体、小分子、ADC、制剂）、临床申报到商业化生产的一站式服务，简化并高效地帮助全球合作伙伴开发高质量的包括ADC在内的生物偶联药物。由于供应链的缩短和各功能块的高效整合，从蛋白质的DNA合成到ADC的临床申报（IND），药明合联通常仅需15个月即可完成，时间大幅缩短。此外，药明合联具有的创新WuXiDAR4™技术平台，可以改善ADC的均一性，从而提高ADC的有效性及安全性。医药观澜：您如何看待“合作”在生物医药创新生态中的作用和影响？具体到XDC研发领域，您认为行业应该如何构建、利用和加速合作，来促进创新？朱梅英博士：生物医药行业的发展离不开产业链上下游各方的合作。以抗体药物为例，该领域的发展就是各个领域相互支持、相互合作的结果。在上世纪90年代，大家刚开始开发抗体的时候，表达量非常低，生产成本很高。过去三十年中，得益于分子生物学等科学领域的发展，以及抗体生产各环节工艺技术的进步和优化，今天抗体的表达量已得到大幅提高，生产成本也随之下降。与传统的抗体类药物相比，XDC类药物的研发难度系数很高，作用机理和生产工艺更加复杂，技术壁垒也高，几乎没有一家公司可以独立完成所有的药物研发和生产工作，所以更需要行业之间的紧密合作，以加速这类药物的研发和创新。药明合联的目标就是希望通过建设开放式、一体化的偶联药物技术平台，加速和变革全球偶联药的发现、开发和生产进程。截至2023年6月30日，药明合联已赋能全球近304家合作伙伴加速研发和生产生物偶联药，完成和在进行中的综合ADC项目（从抗体到ADC制剂）高达110个，赋能完成了47个ADC项目的IND申报，且有16个处于2/3期临床试验的项目。我们相信通过资源的合理利用、协同分工，行业一定可以大大加速XDC产品的研发进程，使它们早日造福更多病患。医药观澜：在XDC领域，您认为未来10年有望迎来哪些变革性的突破？朱梅英博士：首先，对于ADC，我依然非常看好。虽然目前已有十多款ADC获批上市，惠及了很多血液癌症、实体瘤等患者，但每种疗法都有其局限性。随着对肿瘤生物学理解的加深，新靶点的出现、连接子技术的创新，以及更多具有不同作用机制的药物的出现，我相信行业可以开发出具有更好的有效性和安全性的ADC，为那些尚无法从现有ADC获益的患者以及那些对现有ADC产生耐药性的患者提供新的治疗选择。ADC之外，我比较看好的一类新疗法是AOC。目前已有多种siRNA获得了FDA批准，siRNA的发展也得到了越来越多研究人员的关注，但是siRNA递送系统的设计是临床应用的关键问题，它们面临着血清稳定性短、膜通透性低以及缺乏组织选择性等挑战。受ADC概念的启发，抗体-siRNA偶联物（AOC）已成为一种靶向siRNA药物递送的潜在载体。这类疗法能够给siRNA的递送提供靶向性，并延长药物在体内的半衰期，提高其在血清中的稳定性和膜通透性，从而带来更好的疗效和安全性。目前全球已有几家公司在进行AOC类药物的研究，进度最快的已进入2期临床。ARC是另外一类非常有潜力的偶联药物，这类疗法增强了放射性药物的靶向性，不仅可以用于疾病的精准诊断，还可以用于疾病的治疗。此外，多肽药物偶联物、抗病毒偶联药物等XDC也非常具有潜力。总体而言，我认为未来十年在XDC领域有很多进展值得期待。医药观澜：您如何看待当前全球医药的创新趋势？创新机会在哪里？您对未来几年的预判是什么？朱梅英博士：在抗体药物领域，一个备受关注的创新方向是双特异性抗体。双抗可以靶向两个不同的靶点，因此可以产生更好的药效，同时规避一些安全性。另外，双抗也可以用来作为ADC的一个载体，提高ADC的疗效和安全性。此外，ADC和免疫疗法的联用也会是一个趋势。已有非常多的临床试验数据证明，ADC联合免疫疗法（PD-1/PD-L1抑制剂等）非常有效。所以，医药领域的创新，除了分子机制的创新，也会有临床上的联合用药创新。但无论是哪种形式的创新，大家的目标都是一致的，就是让药物的疗效更好，让病人能够得到更好的医治。医药观澜：作为创新药领域的资深人士，您还有哪些观点和声音希望向业界传递？朱梅英博士：众所周知，创新药，尤其是XDC创新药，研发上下游的产业链条很长，很难有一家公司可以建立所有的研发和生产体系。在各方面资源有限的情况下，我认为生态圈各方之间应该术业有专攻，做自己最擅长的事情。创新药公司聚集能量研发出更多好药，一体化平台型公司利用工艺平台优势、放大能力优势、生产能力优势、以及质量体系合规优势加速推进到临床或商业化。大家相互交流和合作，通过优势互补，强强联手，加速创新药的研发和商业化进度，使它们早日问世，救治更多的病患。本文来自药明康德内容团队，欢迎个人转发至朋友圈，谢绝媒体或机构未经授权以任何形式转载至其他平台。转载授权请在「医药观澜」微信公众号留言联系我们。其他合作需求，请联系wuxi_media@wuxiapptec.com。免责声明：药明康德内容团队专注介绍全球生物医药健康研究进展。本文仅作信息交流之目的，文中观点不代表药明康德立场，亦不代表药明康德支持或反对文中观点。本文也不是治疗方案推荐。如需获得治疗方案指导，请前往正规医院就诊。
