Catalytic component of a P4-ATPase flippase complex which catalyzes the hydrolysis of ATP coupled to the transport of phospholipids, in particular phosphatidylcholines (PC), from the outer to the inner leaflet of the plasma membrane (PubMed:25315773, PubMed:17948906). May participate in the establishment of the canalicular membrane integrity by ensuring asymmetric distribution of phospholipids in the canicular membrane (By similarity). Thus may have a role in the regulation of bile acids transport into the canaliculus, uptake of bile acids from intestinal contents into intestinal mucosa or both and protect hepatocytes from bile salts (By similarity). Involved in the microvillus formation in polarized epithelial cells; the function seems to be independent from its flippase activity (PubMed:20512993). Participates in correct apical membrane localization of CDC42, CFTR and SLC10A2 (PubMed:25239307, PubMed:27301931). Enables CDC42 clustering at the apical membrane during enterocyte polarization through the interaction between CDC42 polybasic region and negatively charged membrane lipids provided by ATP8B1 (By similarity). Together with TMEM30A is involved in uptake of the synthetic drug alkylphospholipid perifosine (PubMed:20510206). Required for the preservation of cochlear hair cells in the inner ear (By similarity). May act as cardiolipin transporter during inflammatory injury (By similarity).

The phospholipid flippase ATP8B1 is involved in the pathogenesis of Ulcerative Colitis via establishment of intestinal barrier function.

OBJECTIVE:

Patients with mutations in ATP8B1 develop Progressive Familial Intrahepatic Cholestasis type 1 (PFIC1), a severe liver disease that requires life-saving liver transplantation. PFIC1 patients also present with gastrointestinal problems, including intestinal inflammation and diarrhea, which are aggravated after liver transplantation. Here we investigate the intestinal function of ATP8B1 in relation to inflammatory bowel diseases.

DESIGN:

ATP8B1 expression was investigated in intestinal samples of patients with Crohn's Disease (CD) or Ulcerative Colitis (UC) as well as in murine models of intestinal inflammation. Colitis was induced in ATP8B1-deficient mice with Dextran Sodium Sulphate (DSS) and intestinal permeability was investigated. Epithelial barrier function was assessed in ATP8B1 knock-down Caco2-BBE cells. Co-immunoprecipitation experiments were performed in Caco2-BBE cells overexpressing ATP8B1-eGFP. Expression and localization of ATP8B1 and tight junction proteins were investigated in cells and in biopsies of UC and PFIC1 patients.

RESULTS:

ATP8B1 expression was decreased in UC and DSS-treated mice, and associated with a decreased Tight Junctional pathway transcriptional program. ATP8B1-deficient mice were extremely sensisitve to DSS-induced colitis, evidenced by increased intestinal barrier leakage. ATP8B1 knockdown cells showed delayed barrier establishment that associated with affected Claudin-4 (CLDN4) levels and localization.. CLDN4 immunohistochemistry showed a tight-junctional staining in control tissue, whereas in UC and intestinal PFIC1 samples, CLDN4 was not properly localized.

CONCLUSION:

ATP8B1 is important in the establishment of the intestinal barrier Downregulation of ATP8B1 levels in UC, and subsequent altered localization of tight junctional proteins, including CLDN4, might therefore be an important mechanism in UC pathophysiology.

Simultaneous total internal biliary diversion during liver transplantation for progressive familial intrahepatic cholestasis Type 1: Standard of care?

BACKGROUND:

Post liver transplant (LT) patients with Progressive Familial Intrahepatic Cholestasis Type 1 (PFIC-1) often develop progressive graft steatohepatitis, intractable diarrhea & growth failure. A total internal biliary diversion (TIBD) during a LT may prevent or reverse these adverse events.

METHODS:

Children with PFIC- 1 who underwent a LT at our institute were divided into two groups, A & B based on the timeline where we started offering a TIBD in association with LT. Pre LT parameters, intraoperative details, post transplant complications like graft steatosis and diarrhea also analysed between two groups and their growth velocity were measured in follow up.

RESULTS:

Of 550 pediatric LT performed between 2011 and 2022, 13 children underwent LT for PFIC-1. Group A had 7 patients (A1-A7) and Group B had 6 (B1-B6). Patients A1,A4,B4,B5 had a failed partial internal biliary diversion prior offering them a LT. Patients A1, A2,& A6 in Group A died in the post LT period (2 early allograft dysfunction & 1 PTLD) whereas A3,A4 & A5 had graft steatosis in follow up. A4 was offered a TIBD 4 years post LT following which the graft steatosis fully resolved. In group B, B1, B2, B5, B6 underwent TIBD during the LT and B3 and B4 had it 24 & 5-months subsequently for intractable diarrhea and graft steatosis. None of the patients in group B demonstrated a graft steatosis or diarrhea and had good growth catch up during follow up.

CONCLUSION:

We demonstrate that simultaneous TIBD in patients undergoing LT should be standard practice as it helps dramatically improve outcomes in PFIC-1 as it prevents graft steatosis and/or fibrosis, diarrhea and improves growth catch-up.

Choline is an essential nutrient, and its deficiency causes steatohepatitis. Dietary phosphatidylcholine (PC) is digested into lysoPC (LPC), glycerophosphocholine, and choline in the intestinal lumen and is the primary source of systemic choline. However, the major PC metabolites absorbed in the intestinal tract remain unidentified. ATP8B1 is a P4-ATPase phospholipid flippase expressed in the apical membrane of the epithelium. Here, we use intestinal epithelial cell (IEC)-specific Atp8b1-knockout (Atp8b1^IEC-KO) mice. These mice progress to steatohepatitis by 4 weeks. Metabolomic analysis and cell-based assays show that loss of Atp8b1 in IEC causes LPC malabsorption and thereby hepatic choline deficiency. Feeding choline-supplemented diets to lactating mice achieves complete recovery from steatohepatitis in Atp8b1^IEC-KO mice. Analysis of samples from pediatric patients with ATP8B1 deficiency suggests its translational potential. This study indicates that Atp8b1 regulates hepatic choline levels through intestinal LPC absorption, encouraging the evaluation of choline supplementation therapy for steatohepatitis caused by ATP8B1 dysfunction.

从Odevixibat在国内报产，浅谈益普生管线布局

Mirum’s Livmarli gets second FDA nod for severe itch associated with liver disease

The FDA greenlit Mirum Pharmaceuticals’ Livmarli (maralixibat) for the treatment of cholestatic pruritus in patients aged five years and older with progressive familial intrahepatic cholestasis (PFIC), marking the oral ileal bile acid transporter inhibitor’s second US approval after its initial clearance for Alagille syndrome (ALGS) in 2021.“Livmarli has the potential to have a transformational impact for patients with cholestatic pruritus associated with PFIC, and importantly, offers an option for those patients with the rarest of subtypes,” remarked Mirum CEO Chris Peetz. The company, however, noted that Livmarli is not intended for the treatment of patients with PFIC type 2 characterised by a severe impairment in the bile salt export pump (BSEP) protein.The expanded label approval was based on findings from the Phase III MARCH trial, involving 93 patients with multiple PFIC genotypes, including those with unidentified mutations. In the study, Livmarli demonstrated significant improvements in pruritus, serum bile acid, bilirubin, and growth compared with placebo in a cohort comprising combined genetic subtypes. The application was also backed by favourable transplant-free survival outcomes seen with Livmarli in the Phase II INDIGO study.Livmarli is rivalled by Ipsen’s Bylvay (odevixibat), the latter having secured US approvals for both ALGS and PFIC in 2021. Although Bylvay has an edge in PFIC owing to an earlier approval and a younger eligible population from three months old, Mirum hopes to tackle it by seeking approval of a higher-strength version for younger patients with PFIC, later this year. Alongside the US, Livmarli has been approved for ALGS in the EU, Canada and other countries. The drugmaker has also filed with the European Medicines Agency for approval in patients with PFIC aged two months and older. Livmarli generated sales of $141.8 million last year.

FDA approves Mirum’s LIVMARLI for cholestatic pruritus

Mirum Pharmaceuticals’s LIVMARLI (maralixibat) oral solution is now available in the US for cholestatic pruritus in patients with PFIC. Credit: Mirum Pharmaceuticals / Business Wire. The US Food and Drug Administration (FDA) has approved Mirum Pharmaceuticals ’s LIVMARLI (maralixibat) oral solution to treat cholestatic pruritus. The medication is intended for patients aged five years and above with progressive familial intrahepatic cholestasis (PFIC). A once-daily, orally administered ileal bile acid transporter inhibitor, LIVMARLI is the sole FDA-approved treatment for PFIC. The approval is based on findings from the Phase III MARCH clinical trial in 93 PFIC patients. The randomised trial enrolled people with PFIC genetic types PFIC1, PFIC2, PFIC3, PFIC4 and PFIC6, along with people with unidentified mutational status. In addition to its current approvals, Mirum has filed a supplemental new drug application to the FDA. The aim is to introduce a higher concentration formulation of LIVMARLI, which was utilised in the Phase III MARCH study. This could expand the treatment’s label for younger PFIC patients later in 2024. A rare genetic disorder, PFIC results in progressive liver disease that can go on to cause liver failure. Mirum Pharmaceuticals CEO Chris Peetz stated: “LIVMARLI has the potential to have a transformational impact for patients with cholestatic pruritus associated with PFIC, and importantly, offers an option for those patients with the rarest of subtypes. “Thank you to the patients, families and clinicians whose study participation made this approval possible.” In September 2023, the company concluded the acquisition of the bile acid product portfolio of Travere Therapeutics . Mirum gained the rights and assets related to Cholbam (cholic acid) and Chenodal (chenodiol). Travere secured $210m from Mirum, with the potential to receive $235m in sales-based milestone payments.
