Confirmatory Study to Examine Negative Predictive Value (NPV) and Safety of Skin Testing With PRE-PEN, a Minor Determinant MIxture (MDM) of Penicillin Antigens, and Amoxicillin Reagent Against an Oral Challenge With Amoxicillin

A self- or parent-reported history of penicillin allergy excludes approximately 10% of the US population from receiving penicillin (Kerr 1994, Kagy, Blaiss 1998, Solensky et al. 2000, Neugut et al. 2001). Yet approximately 80% of patients labeled as allergic to the penicillins can safely take these antibiotics without fear of a life-threatening reaction (Gadde et al. 1993, Macy et al. 1997). The outcomes of this erroneous classification of patients include unnecessary denial of an effective and well-tolerated class of antibiotics, which are often the treatment of choice.
The primary aim of skin testing with the Penicillin Skin Test Kit is to identify subjects at very low risk of developing acute IgE-dependent reactions when given a penicillin or cross-reacting drug. The negative predictive value (NPV) of skin testing is assessed by oral challenge with a penicillin.

Relation of Streptococcus Pyogenes tonsillitis isolate to antimicrobial agents and its infection treatment

We reported the case of tonsillitis treatment in a 17-years-old boy with use of chemical non-antibiotic preparations, plant derived products and antibiotic benzathine phenoxymethylpenicillin. The antimicrobial agents for treatment were selected on the basis of their activity against a disease agent, the group A β-hemolytic strain Streptococcus pyogenes BS1 isolated from a patient. The bacterium was susceptible in vitro to β-lactams, with largest zones conditioned by penicillin G and benzathine phenoxymethylpenicillin discs, to fluoroquinolones and to cephems, with exception of cefazolin. Lincosamide clindamycin, macrolide spiramycin, aminoglycoside gentamicin, erythromycin, tetracycline and combination of sulfamethoxazole and trimethoprim were inactive against this bacterium. The Streptococcus pyogenes BS1 demonstrated intermediate susceptibility to the cephalosporin cephalexin, fluoroquinolone lomefloxacin and glycopeptide vancomycin. Non-antibiotic preparations were evaluated against Streptococcus pyogenes BS1 also. Among them “Stomatidin”, “Chlorophyllipt”, and phages of “Pyofag” were more effective than “Decatylen”, “Decasan” and “Furadonin” in vitro. The antimicrobial applications of “Stomatidin”, “Chlorophyllipt” and phages of “Pyofag” in the patient were less effective compared to the result of antibiotic benzathine phenoxymethylpenicillin treatment. Complete recovery of the patient was achieved with use of this antibiotic and Calendula flower extract as an local anti-inflammatory agent.

Relative Bioavailability of Phenoxymethylpenicillin Preparations in a Cross‐Over Study

Abstract In a cross‐over study on 6 healthy volunteers, the serum concentrations and the bioavailability of 16 oral phenoxymethylpenicillin preparations have been compared. The dissolution rates of the tablets and capsules were studied. The preparations included 5 aquous solutions, 7 tablets or capsules, and 4 suspensions. The highest bioavailability was seen with the aquous solutions and the tablets or capsules containing the potassium salt of phenoxymethylpenicillin. Two calcium salt tablet brands showed lower serum mean concentrations and bioavailability than the other, potassium salt, tablets and capsules, but the differences were only statistically significant for one of the calcium salt tablets as compared to the tablet with the highest mean concentrations. The poorer bioavailability of the calcium salt tablets was reflected in longer dissolution times. The suspensions intended for paediatric use were significantly inferior to most of the other preparations. The lowest bioavailability was observed with a suspension containing the poorly soluble benzathine phenoxymethylpenicillin.
