ChemoCentryx：阿伐可泮 (Avacopan)靶点：C5aR （推荐阅读：GPCR3D靶点调研：C3aR和C5aR与炎症性疾病）适应症：抗中性粒细胞胞浆自身抗体相关性血管炎 阿伐可泮 (Avacopan, Tavneos) 是生物制药公司ChemoCentryx发现和开发的一款首创的口服选择性补体5a受体 (C5aR) 拮抗剂。该药适用于成人患者的抗中性粒细胞胞浆自身抗体(ANCA)相关性血管炎的辅助治疗。抗中性粒细胞胞浆自身抗体相关性血管炎（AAV）是一种罕见的（估计发病率为每年十万分之三）"寡免疫性"系统性小血管炎，其典型特征是血清中存在ANCAs。AAV 的整个病程包括肉芽肿伴多血管炎（GPA）、显微镜下多血管炎（MPA）、嗜酸性粒细胞肉芽肿伴多血管炎（EGPA）和药物诱发的 AAV。尽管病理生理学很复杂，但过去约 20 年的研究已确定替代补体途径的关键作用，特别是 C5a 与 C5aR 受体在 AAV 中的相互作用。Avacopan 最早于 2021 年 9 月 27 日在日本获批上市，随后先后在美国、欧盟、加拿大和澳大利亚获批上市，用于治疗抗中性粒细胞胞浆抗体相关性血管炎、肉芽肿伴多血管炎和显微镜下多血管炎的成年患者。除此之外，靶向多项其它临床适应症，如化脓性汗腺炎和C3肾小球病等的研究工作正在进行中。3. 辉瑞：硫酸瑞美吉泮 (Rimegepant Sulfate)靶点：CGRP 受体适应症：偏头痛 (推荐阅读：偏头痛：CGRP相关新药研发)Rimegepant 是一款口服 CGRP 受体拮抗剂，用于急性治疗成人伴有或不伴有先兆的偏头痛。Rimegepant 最初由 BMS 团队开发(BMS-742413)，2016 年新成立的生物技术公司 Biohaven Pharmaceutical 从 BMS 获得 Rimegepant 的全球独家开发和商业化权利。Biohaven 团队围绕 Rimegepant 进行了一系列的临床研究和分析，并开发出衍生的鼻喷雾剂偏头痛药物 Zavegepant。根据中国药物临床试验登记与信息公示平台 (http://www.chinadrugtrials.org.cn/) 公示信息，Biohaven 公司已在中国注册3项临床试验研究，其中2项均已完成。2022年5月，辉瑞宣布斥资总计约116亿美元的现金收购 Biohaven (全球首款治疗急性偏头痛的鼻喷雾剂（辉瑞）)。Rimegepant 最早于 2020 年 2 月 27 日在美国获批上市，随后先后在以色列、阿联酋、科威特、欧盟和冰岛等国获批上市。目前，该药物正在进行慢性鼻窦炎、鼻息肉和三叉神经痛等临床试验研究。4. 远大医药 ： RYALTRIS (莫米松糠酸酯/盐酸奥洛他定 (Olopatadine Hydrochloride/Mometasone Furoate Monohydrate))靶点：H1R + GR适应症：季节性过敏性鼻炎RYALTRIS（莫米松糠酸酯/盐酸奥洛他定） 是一款由印度药企 Glenmark 开发的新型复方处方鼻腔喷雾剂。该药同时具有激动糖皮质激素受体 (GR) 和拮抗组胺 H1 受体 (H1R) 的功效。Mometasone 是一种中等效力的合成皮质类固醇，作为糖皮质激素受体激动剂发挥作用，具有抗炎、止痒和收缩血管的特性。Olopatadine 是一款组胺 H1 受体拮抗剂，用于治疗过敏性结膜炎和鼻炎。因为组胺是引起炎症和过敏反应的主要炎症介质，Olopatadine 通过阻断组胺受体的作用而起作用。与其他抗过敏眼科药物相比，Olopatadine 不会对细胞膜造成干扰，因此具有良好的舒适性和耐受性。RYALTRIS 最早于2019年在澳大利亚获批上市，随后先后在南非、英国、美国、加拿大等国获批上市。2019 年，远大医药与 Glenmark 订立了独家授权协议，内容为Glenmark授权远大医药为中国独家销售 Ryaltris。Glenmark将负责制造和供应授权产品，而远大医药负责授权产品于中国的临床试验及注册工作，并在获批准上市后可以于中国独家销售授权产品20年。目前，全球有多款同时靶向 H1R + GR 的复方型临床候选药物处于临床研究阶段。布局该方向的不乏顶级药企，如葛兰素史克 (GSK)。5. 安进: 依瑞奈尤单抗 (Erenumab) (推荐阅读：第一款靶向GPCR的抗体药：Aimovig® (erenumab))靶点：CGRP 受体适应症：偏头痛 Erenumab 是一款由安进公司开发的人源单克隆抗体，也是 FDA 批准的首款靶向 GPCR 的单克隆抗体药物 (推荐阅读: erenumab (AMG 334) 的作用机制研究)。Erenumab 能特异性结合和拮抗降钙素基因相关肽受体 (CGRPR)，从而预防偏头痛。2017年4月，安进宣布与诺华就 Erenumab 扩大商业合作，该药物正被研究用于预防偏头痛。这项扩大的商业合作建立在诺华和安进公司于2015年在阿尔茨海默病和偏头痛领域建立的全球神经科学合作的基础上。诺华于 2019 年在中国启动两项临床研究，用于评估 Erenumab 注射液在偏头痛治疗方面的安全性和疗效。Erenumab 最早于 2018 年在美国获批上市，随后先后在澳大利亚、冰岛、挪威、瑞士、加拿大和日本等国获批上市。除偏头痛治疗外，该药物正在进行包括头痛、潮热 (推荐阅读: 首款NK3 受体拮抗剂：FDA 批准Fezolinetant用于治疗更年期潮热)、颞下颌关节功能紊乱综合征、稳定型心绞痛等等适应症的临床试验研究。6. 协和麒麟: 伊万卡塞 (Evocalcet)靶点：CaSR适应症：继发性甲状旁腺功能亢进症Evocalcet 是由日本协和麒麟制药开发的一款钙敏感受体 (CaSR) 激动剂，用于治疗高钙血症、继发性甲状旁腺功能亢进症等。协和麒麟已在中国申请进行 2 项关于 Evocalcet 的临床试验研究，用于评估该药对治疗慢性肾脏病维持性透析患者的继发性甲状旁腺功能亢进症。其中一项由广东省人民医院余学清教授牵头的 “KHK7580在继发性甲状旁腺功能亢进症患者中的Ⅲ期临床研究” 已完成。截至目前，Evocalcet 仅在日本获批上市。7. 大冢制药: 依匹哌唑 (Brexpiprazole)靶点：5-HT1A + 5-HT2A + D2R适应症：抑郁症Brexpiprazole 是一款由日本大冢制药开发的一款血清素-多巴胺活性调节剂，用于辅助治疗重度抑郁障碍、精神分裂症和阿尔茨海默病引起的痴呆症相关的躁动。Brexpiprazole 是一款非典型抗精神病药，也是一种新型 D2 多巴胺和血清素 1A 部分激动剂，被称为血清素-多巴胺活性调节剂。它对血清素、多巴胺和 α 肾上腺素能受体有很高的亲和力。Brexpiprazole 在结构上与另一款非典型抗精神病药物 Aripiprazole 非常相似，但 Brexpiprazole 与多巴胺和血清素受体的结合亲和力不同。与Aripiprazole 相比， Brexpiprazole 产生部分激动剂介导的锥体外系症状等不良反应的可能性较小，这是因为它在 D2 受体上的内在活性较低。大冢制药已在中国申请 3 项临床试验研究，其中由首都医科大学附属北京安定医院姜涛牵头的有关抑郁症治疗的 “Brexpiprazole片剂单次给药的药代动力学及耐受性和安全性研究” 已完成。Brexpiprazole 最早于 2015 年在美国获批上市，随后先后在加拿大、日本和欧盟获批上市。除抑郁症治疗外，该药物正在全球范围内进行包括躁动、精神分裂症、阿尔兹海默症和表现焦虑等多项适应症的临床测试研究。8. 参天制药: Tapcom (他氟前列腺素/马来酸噻吗洛尔 (Tafluprost/Timolol Maleate))靶点：PTGFR + βAR适应症：青光眼、高眼压症Tapcom 是由日本参天制药开发的一款他氟前列腺素 (Tafluprost) 和马来酸噻吗洛尔 (Timolol) 的复方滴眼液。Tafluprost 是一种眼科前列腺素类似物，用于降低眼压过高或开角型青光眼患者的眼压。从化学角度看，Tafluprost 是前列腺素 F2-α 的氟化类似物。Timolol 是一种非选择性β肾上腺素能受体拮抗剂，用于治疗眼压过高或开角型青光眼的眼压升高。日本参天制药先后开发了多款滴眼液产品，并在中国申请进行临床试验研究。其中，Tapcom (DE-111A) 相关研究由复旦大学附属眼耳鼻喉科医院孙兴怀教授牵头进行，评估该药物对开角型青光眼或高眼压症的疗效和安全性，题为 “评价DE-111A滴眼液的有效性和安全性的研究”，该项目已完成。Tapcom 于2013年首次在日本获批上市，其后先后在冰岛、法国、波兰、德国等国获批上市。除 Taqcom 外，全球范围内围绕 PTGFR + βAR 进行青光眼和高眼压症治疗的药物还包括以下几项：9. 武田制药: 替度鲁肽 (Teduglutide)靶点：GLP-2R适应症：短肠综合征Teduglutide 是由英国罕见病制药巨头 Shire Development 开发的一款胰高血糖素样肽-2（GLP-2）类似物，用于治疗需要肠外营养支持的短肠综合征（SBS）患者。Teduglutide 由 33 个氨基酸组成，使用经 DNA 重组技术改造的大肠杆菌菌株制造。Teduglutide 与 GLP-2 相差一个氨基酸（丙氨酸被甘氨酸取代）。这一替代的意义在于，Teduglutide 比内源性 GLP-2 的作用时间更长，并更能抵抗二肽基肽酶-4 的蛋白水解作用。Teduglutide 最早于 2012 年在欧盟获批上市，随后分别在美国和日本获批上市，用于治疗短肠综合症。2019年，武田制药豪掷460亿英镑收购 Shire Development，被称为医药界的“蛇吞象”并购，这使得武田制药跻身全球十大生物制药公司。Teduglutide 被用于临床治疗克罗恩病和肾脏疾病的临床研究也因被收购原因终止。10. 阿斯利康: Duaklir Genuair (阿地溴铵/富马酸福莫特罗 (Aclidinium Bromide/Formoterol Fumarate))靶点：M3R + β2AR适应症：慢性阻塞性肺疾病Duaklir Genuair 是由阿斯利康开发的一款复方阿地溴铵 (Aclidinium Bromide)/富马酸福莫特罗 (Formoterol Fumarate) 吸入干粉剂。该药用作一种维持性支气管扩张剂，以减轻慢性阻塞性肺病成人患者的疾病症状。其中，Aclidinium bromide 是一种新颖的长效毒蕈碱拮抗剂，Formoterol fumarate 是一种长效β2受体激动剂。Duaklir Genuair 通过干粉吸入器 Genuair (下图)给药，每日2次，是首个与单药相比使呼吸喘促表现出统计学意义显著改善的复方药。Duaklir Genuair 最早于 2012 年在欧盟获批上市，随后于2019年在美国获批上市，用于治疗慢性阻塞性肺疾病。以上文章来源于GPCR drug diacovery，作者段思研       本文仅作信息交流之目的，文中观点不代表相关疾病治疗立场，也不是治疗方案推荐。如需获得治疗方案指导，请前往正规医院就诊。
